Identify different tracheostomies across the spectrum of populations (ie transplants vs trauma) to compare and contrast indications/treatment plan.
Describe different mechanical ventilation modes, types and settings as well as determine patient’s candidacy for speaking valve use on a ventilator.
Summarize literature related to dysphagia and communication deficits associated with tracheostomies in different populations.

15-30 minutes : Tracheostomies Population Specific- (Compare and contrast trauma vs transplant)

Speaker Bio and Disclosures:
Rachael Silveira is an ASHA licensed/credentialed Speech- Language Pathologist currently working at a Level 1 Trauma/Transplant Hospital in Miami specializing in dysphagia and communication disorders, with diverse training in dysphagia management associated with respiratory compromised/artificial airways and medically complex neurogenic populations.
Rachael strives to be a lifelong student by earning yearly ACE (awards of continuing education) recognition and continually supporting the SLP community by providing clinical mentorship as well as providing support for Medical SLP platforms to promote continued growth for SLPs. Rachael has made patient advocacy and improving overall patient quality of life a personal mission and goal. She has spearheaded a robust multidisciplinary Tracheostomy Teams in multiple facilities with a mission of providing exceptional care to the population by utilizing standardized protocols to streamline care. In addition, Rachael has curated multiple avenues for education for SLPs including a “Badge Buddy Series” and an open collaborative called TRACHtalks, for promoting tracheostomy evidence-based research in an accessible manner.
As our field is growing rapidly Rachael focuses on continued efforts to improve medical SLP education with ongoing participation in national and international conference lectures, active research, and guest lectures to multiple universities and hospitals.

Speaker Information and Disclosures
Oneida Chi, MS, CCC-SLP (they/ them) is a first-generation Chinese American, genderqueer licensed speech-language pathologist from San Francisco. Oneida has worked in schools preK- 12th grade throughout the Bay Area for the last fifteen years. In 2016, they started their private practice, Bay Area Outspoken Speech Services, to work on their voice and assist friends and community members with exploring their voices. Currently they work at the Regional Center with toddlers 0-3, the Gender Health Clinic in Santa Clara, supervise the Gender Affirming Voice Clinic at San Francisco State University, and serve on the CSHA Board Committee of Diversity and Inclusion.
Sandy Hirsch, MS, CCC-SLP (she/her) is a cisgender speech-language pathologist (SLP). She has an international reputation in gender affirming voice and communication training. She has co- edited and authored all editions of the Plural Publishing textbook on voice and communication training for TGNC clients (Adler, Hirsch & Mordaunt, 2006, 2012; Adler, Hirsch & Pickering, 2019). Her work has been featured across all media, Stemple & Hapner, 5th ed. and ASHA’s Perspectives. Ms. Hirsch is a presenter at Esprit and Gender Odyssey, and has presented at ASHA, CPATH, the Voice Foundation Symposium, VASTA, Fall Voice, the Art and Science of Performing Voice and MTHF in Victoria, BC. She has lectured at the UW, WWU, and SUNY, New Paltz. Ms. Hirsch is owner of Give Voice in Seattle.

June 18, 2024 Discourse Level Treatment for Aphasia
Discourse-level treatments for aphasia that show promise in improving word and sentence production in discourse have recently emerged in the aphasia research literature (Dipper et al., 2021). By focusing on both microstructure (the specific words, phrases, and sentences used in the discourse) and macrostructure (the organization of the discourse as a whole), discourse-level treatments aim to help clients with aphasia speak more fully in everyday life (Armstrong, 2000; Dipper et al., 2021). This resource is an intermediate-level resource for speech-language pathologists who are interested in learning more about discourse-level treatment and highlights a variety of discourse-level treatment approaches that clinicians may consider when working with clients with aphasia.

May 27, 2024 ICU Extubation Protocol
After extubation, patients are at high risk for silent aspiration (Brodsky et al., 2017; Marvin et al., 2018; McIntyre et al., 2021; Scheel et al., 2015). However, a swallow study is not always practical or possible. The SLP is responsible for determining when one is needed and if oral (PO) intake can be started without or until one can be completed.
Starting PO too early can increase the risk of aspiration pneumonia, while starting too late may lead to unnecessary NPO status or feeding tube placement (Brodsky et al., 2017; Hongo et al., 2022; Macht et al., 2011; Marvin, 2022; Wallace & McGrath, 2021). Since there are risks at both ends of the spectrum, the stakes can be high in determining when to begin PO intake and whether or not to pursue an instrumental swallow study.
Below is a list of relevant questions to help the SLP determine where the patient falls on an informal risk spectrum ranging from high to low. This information will help the SLP decide when to pursue or defer an instrumental swallow study.

Valeria Gary, M.A., CCC-SLP owns Southeast Parkinson’s Speech Services LLC, where she provides maintenance speech therapy services. She also owns Total Body Cognition LLC, empowering those living with chronic conditions to minimize symptoms and thrive through nutrition, physical activity, and education. Valeria holds certifications in Speech-Language Pathology, Personal Training, Brain Health Training, Behavior Change, Nutrition Coaching, and Health Coaching. A member of Allied Team Training for Parkinson’s and an advisory board member for Parkinson’s Foundation Georgia, Valeria has a particular interest in wellness for people living with Parkinson’s Disease.
At Wellstar Kennestone Outpatient Neurorehabilitation Clinic, Valeria provides clinical services, supervises graduate students, and co-hosts a monthly Parkinson’s support group. Her specialty certifications include LSVT-LOUD, LSVT LOUD for Life, Speak OUT!, McNeill Dysphagia Therapy Program, and MBSImP (Modified Barium Swallow Impairment Profile).
As a mentor for the Medical SLP Collective, Valeria assists Speech-Language Pathologists in providing evidence-based care in the areas of Parkinson’s Disease, motor speech disorders, and cognition.
