262 – Acute SLPs: Prepping Patients for the Next Steps – Alicia Brooms, MS, CCC-SLP

Listen Later

Your patient is set to discharge at the end of the week, but you know their needs are high and their level of support at home won't be. How do you help ensure they're going to be prepared to go home and not get lost in the shuffle? How can SLPs best care for our patients after they're no longer under our care? My guest this week is sharing the answers to these questions and more!

In this episode of SYP, I have a conversation with Alicia Brooms, MS, CCC-SLP, about the weight our recommendations hold and the best ways we can prep our patients for the next steps and the next level of care. Alicia is passionate about considering the patient's big picture, making recommendations that help the patient with more than what's on their plate, and adequately preparing them for the next stage of their recovery and return to their typical lives. She is a champion for interdisciplinary care and partnering with our case management and social work colleagues to ensure no patients are left to figure it all out on their own!

In this episode, you'll hear about:

*Relationship building with case managers and social workers
*Enhancing goals/recommendations by using the Family-Centered Approach
*Helping unfunded patients
*Preparation patients for returning to work/school
