A hormone clinic owner will spend $75,000 on a piece of equipment without blinking. Ask that same owner to spend $2,000 a month getting found, and suddenly there's a budget conversation.
Yuri Elkaim has spent 20 years watching that exact pattern play out — and this episode is him taking it apart, piece by piece, with the numbers to back it up.
Yuri is a former professional soccer player, a New York Times bestselling author, and the founder and CEO of Healthpreneur. He built and sold an online health business that reached more than 500,000 people and put him on Dr. Oz and The Doctors. He now works exclusively with health practitioners on the side of the business nobody taught them: acquisition, offer design, pricing, and leverage.
Jody brought him a list of challenging questions. He answered all of them, and then some.
Why isn't word of mouth a real marketing strategy?
Because, in Yuri's framing, a practice that runs entirely on referrals hasn't built anything repeatable. He calls it a good service that people happen to talk about — not a business. There's no system you can turn up, no lever you can pull when a slow month arrives. His harder point: refusing to market isn't humility. If you're good and nobody knows you exist, patients choose someone worse, and you're the reason.
How much should a cash-pay clinic spend on marketing?
Yuri argues the question itself is backwards. A budget is what you need when marketing doesn't work. When it does work, the spend funds itself. Jody gives him a real scenario — a hormone practice six months open, self-funded, $1,000 to $1,500 a month — and he walks the actual math live: patient value around $3,000 a year, three to five year retention, roughly $15,000 lifetime value. From there he backs into what has to be true for the model to hold, including collecting enough cash in the first 30 days to keep buying patients.
Should health practitioners be on Instagram or YouTube?
Yuri is blunt: Instagram is a billboard. It's good at getting attention and that's roughly where it stops. He tells the story of a psychiatrist with posts in the millions of views that generated almost nothing, and explains the distinction he now builds his whole business around — Meta is excellent at letting people know you exist, YouTube is what helps them decide why they should choose you. He also concedes the one thing Instagram does better than any other platform, which is how this interview happened in the first place.
What's actually stopping most practitioners from growing?
Not knowledge. Yuri names the wounded healer pattern — the practitioner who got into this because of their own health story, who then can't say no, cuts prices so more people can afford it, and burns out working one-on-one with no leverage. Plus the line that stopped Jody cold: the people who need your help the most very often want it the least.

Let me put real math to this, because I think most people run this calculation backwards.
Setting up a CRM properly — writing the campaigns, building the logic, mapping the segments — let's call it 60 hours. One time. That's it. Sixty hours and it's built.
Now the manual version. Let's be generous and say you only spend two hours a week keeping track of who needs a follow-up, who's due for a check-in, who came to the event in March and never called back. Two hours a week is 104 hours a year.
So even in year one, the system wins. Sixty versus 104. You hit break-even at week 30 — about seven months in.
Year two? The system costs you zero hours. The manual version costs you another 104.
By the end of year three, you have spent 60 hours total. The clinic doing it by hand has spent 312. That's a 252-hour difference. Six and a half full work weeks. And if you're paying someone twenty dollars an hour to do that tracking, you've spent about five thousand dollars more — to get a worse result.
Because here's the second cost, and it's the bigger one. The manual version stops happening. Not because your people are lazy. Because it's a Tuesday, and there's a patient in room three.

You added another zero to your revenue this year. Your paycheck looks exactly the same. What is going on?
Craig Dacy has an answer, and it isn't the one your accountant gave you.
Craig spent ten years teaching fourth grade before he and his wife paid off just over $30,000 of debt in twelve months — on a teacher's salary. That experience turned into a decade-long career coaching business owners through the money conversations nobody trained them to have. At twenty-seven, Dr. Jordan Wiggins had $200,000 in student debt, no business history, and no bank willing to loan her a dollar. She bootstrapped a women's health clinic out of a single rented room anyway — and grew it into a seven-figure practice that patients flew in from the UK to visit. No paid ads. No Instagram. Word of mouth, until she had to stop taking new patients.
Then she closed it. On purpose.
Jordan is a naturopathic doctor, the founder of The Pleasure Collective, host of the Pleasure Principles podcast, and the author of The Pink Canary, featured in the Washington Post and Cosmopolitan. In this conversation she walks Jody through the whole arc — the build, the ceiling, the exit, and the rebuild.
The business lessons land hard because they're specific. When Jordan became the bottleneck in her own clinic, she didn't hire her way out of it — she set boundaries, raised her prices, and offloaded everything that didn't require her hands. She niched every provider on her team so each one owned a specialty, and her front desk triaged new callers to the best-fit practitioner instead of defaulting to her. She's also candid about the five years she spent pouring her own time, money and equity into team members who were never going to be a fit.
Then there's the clinical thread. Jordan's own teenage years — dismissed pelvic pain, two surgeries, blood work that showed nothing, and doctors who eventually suggested it was psychiatric — are the reasons she built a different standard of care. And years later, prescribing estrogen and watching libido stay flat is what led her to write The Pink Canary and start asking her patients a question most intakes never touch.
Jordan also speaks openly about the season that forced her to stop entirely: a new baby, a two-year pandemic shutdown in Canada, and an abusive relationship she's since rebuilt her life out of. Most clinics don't have a lead problem. They have a "we gave up on them too soon" problem.
Every time a prospect ghosts a phone call, has a great conversation and doesn't book, or says "thanks, I'm not interested" — that lead gets quietly written off, and the clinic goes right back out to buy a new one. In episode three of our six-part series on the marketing and sales gaps inside hormone and functional medicine practices, Jody makes the case that the money you're chasing is already sitting in your CRM.
She walks through the five places leads go to die, and exactly what to do about each one: why nobody answers the phone anymore (and the three personal touches she and Patrick use instead — a voice memo, a 90-second video, and a real email), why a prospect who asks a hundred questions and still doesn't book is almost always just further up the buying journey than you assumed, and why the person who says no in the first thirty seconds is usually saying no to being sold to, not no to you.
Then she gets into the part most clinics skip entirely: educational nurturing. Your prospects know about two percent of what you know about hormones.
