    We are joined by Dr. Seth Collings Hawkins and paramedic David Fifer who come on the show to talk about their new venture the RAW Medicine Podcast. The show is a podcast about providing medicine off the grid. You can also connect with them via their Facebook page for the RAW Medicine Podcast.
    David and Seth saw a need to talk on a podcast about the specific delivery of medicine in those “raw” environments off the grid. They discuss topics like the difference between wilderness EMS and wilderness medicine. A good way to think about RAW medicine is as “low-resource medicine.” You can also follow what they’re doing on their Facebook page for the RAW Medicine Podcast, too.
    What is Wilderness Medicine? It is defined in the new Wilderness Medicine textbook as:
    “Medical care delivered in those areas where fixed or transient geographic challenges reduce availability of, or alter requirements for, medical or patient movement resources.” -Auerbach’s Wilderness Medicine 7e -NAEMSP textbook 2e -Wilderness EMS 1e.
    Wilderness EMS: “Wilderness EMS is the systematic and preplanned delivery of wilderness medicine by formal health care providers.”

  • Disaster NGO Response with Burners Without Borders
    In this episode, we have Dr. KK Pandya (DrPandya.org) on the show to chat about his work with the Burning Man community and their disaster response team called Burners Without Borders. He started off his medical career as a dispatcher for EMS, then as an EMT, and eventually going to medical school.
    In 2005, he was volunteering with the Black Rock, NV emergency services organization as a dispatcher. The burning man community rallied to send a support team to assist with hurricane Katrina response. That was the beginning of the disaster relief organization Burners Without Borders.
    KK went on to share about his experience during Hurricane Irma where he helped operate a 240 bed special needs center located in a local middle school auditorium. He talks about the need for professional dexterity and putting away egos so the whole team focuses on getting the job done.
    3D Printing and Medical Education
    In the second half of the episode, KK talks about his groundbreaking work on 3D imaging in clinical settings (CT and MRI scans). He takes the 3D image data set and is able to create a 3D printable model of the anatomy for demonstrations with patients and for training of providers.
    He’s currently involved in a project to send 3D printed models (based on CT and MRI scans) for practice of cricothyroidotomy (surgical airway) to medical providers in a NATO field hospital in Afghanistan. The intent is to train onsite medical providers how to perform this high stakes procedure on an anatomically accurate model. I am collaborating with an active duty US Army physician as well as several Veterans who have since entered into medical training as physicians.

  • Cadaver Donation and Sensationalistic News Coverage
    In this episode, we talk cadavers and training in healthcare. We bring Dr. Joe Holley on the show to talk about a series of articles from Reuters news agency about the body donation system in the U.S. Joe was disheartened by the sensationalized nature of the articles and the challenges created by the difference between the article and what Joe’s experiences in real life applied science situations.
    Suggestions for Responsible Cadaver Donation:

    * 1) First thank the person for considering it
    * 2) Look for a reputable entity – a university,
    * 3) Ask for references – from loved ones of past donations and from professionals who utilize donations.
    * 4) While there are bad actors out there, the vast majority are professional and treat your donation appropriately.

    Before we touch on the cadaver stories, co-hosts Jamie Davis and Sam Bradley talk briefly about reports of tsunami alerts along the U.S. west coast following a significant undersea earthquake off the coast of Kodiak, Alaska. Though the alerts were later canceled as the predicted effects never materialized, it was still a good example of how the tsunami alert system works along that coastline. Sam referred to the Alaskan earthquake in 1964.

    Dr. Gustavo Flores stopped by our podcast booth at EMS World Expo and sat down to share some of his experiences riding out the storm for Hurricane Maria at his home in Puerto Rico back in September. He also talks about getting out and responding to medical emergency calls in and around his community in the hurricane’s aftermath.

  • Carribean Hurricane Irma Strike with Heather Langner
    On the show this week, we chat with medical student Heather Langner who was moving into her apartment in the Caribbean at St. Maarten island where she attended medical school at the American University of the Caribbean. While getting settled with the help of her mother, Hurricane Irma came through and struck the Virgin Islands.
    As the hurricane struck, the officials on the island put out a call for anyone with medical training such as EMTs, nurses, doctors and other health professionals. They set up areas for patient treatment even though they had very limited supplies from the medical school.
    Winds from Hurricane Irma reached sustained levels of 180 miles per hour with gusts above 220. The eyewall of the storm passed over the school allowing a brief respite to evacuate partially collapsed buildings to the central school building in which most people sheltered.
    After the storm passed, they opened up the doors and started taking in neighbors from nearby communities who needed treatment for mostly minor trauma injuries. Later they were evacuated back to the U.S. mainland by military transport planes.

  • Memphis Community Paramedic Roles in Disasters
    In this episode, we join our friends at Paragon Medical Education Group on the road in Tunica, MS. with an agricultural experience cadaver lab. We talk a little about what they’re teaching in Mississippi then we segue into talking about Memphis Fire Department’s new community paramedic model.
    On the call from Paragon’s team were:

    * Chief Andrew Hart – Memphis EMS Chief
    * Lt Amanda Bowen – Memphis EMS Training

    We start out with Chief Hart talking about the impetus for the ambulance services in Memphis to adjust their model to provide care in a different way. Rapid Assessment, Decision, and Redirection (RADAR) is how they focused their changed system. They have two chase cars with a paramedic from Memphis Fire Department and a Physician from Resurrection Health in Memphis to go out and treat non-emergent patients based on information from dispatchers in the 911 call center.
    Part of the dispatch call center team includes nurses to help with telephone triage of patients who call 911 but qualify as non-emergency patients based on the standard call algorithms. The nurses then help refer the patients to other resources including the RADAR chase cars.

    Daniel Zehner from the DesignSafe Radio podcast joins us on the Disaster Podcast this week. He highlights the research opportunities at NHERI for the major hazard areas they look into: wind, earthquakes, tsunami, coastal erosion, and rapid response after events.  His podcast program focuses on the important work of the team at the Natural Hazards Engineering Research Infrastructure (NHERI). NHERI is committed to making sure the next natural hazard doesn’t have to be a disaster for you and your family.
    Also on the show this week are co-hosts Sam Bradley and Jamie Davis, the Podmedic. We also had our disaster weather expert Kyle Nelson on along with wilderness EMS educator Kevin Reiter from https://wildsafety.com.
    Daniel is the Facility Scheduling and Operations Coordinator for the Natural Hazards Engineering Research Infrastructure (NHERI) Network Coordination Office (NCO). I schedule research projects for scientists doing research in the 8 experimental test facilities of NHERI, and oversee the daily operations of the NHERI NCO and our reporting to the National Science Foundation (NSF).
    As part of his role, he started DesignSafe Radio as a way to help educate the public about natural hazards research and connect with new researchers and practitioners that may be interested in doing testing at one of their research facilities around the country.
