  • (English) Episode 89-Injury Proof Your Legs - The Science of Preventing Lower Body Sports Injuries

This systematic review and meta-analysis aims to investigate the effects and differences of various hamstring eccentric training protocols for the prevention of lower limb injuries, and we further propose a more refined hamstring eccentric training protocol for the prevention of lower limb injuries.

A total of 23 randomized controlled trial (RCT) studies were found to be included in the meta-analysis, and 15 of these trials, totaling 14,721 patients, were determined to be included in the overall lower extremity injury prevention effect.

The analysis showed that the implementation of a hamstring eccentric training program reduced lower extremity injuries by 28%, and it resulted in a 46% decrease in hamstring injury rate and a 34% decrease in knee injury rate.

The subgroup analysis revealed that the frequency of exercise was most significant in the twice-a-week exercise group, that the exercise program was most effective in preventing injuries in the 21–30-week exercise period, and that the program was most effective in preventing injuries in elite athletes and amateur adult athletic populations, compared with adolescents.

  • (English) Episode 88-Navigating Femoroacetabular Impingement - From Diagnosis to Return to Play

Femoroacetabular impingement (FAI) is a pathomechanical process whereby abnormal contact between proximal femur and acetabulum at end range of hip motion induces chondrolabral lesions within the hip joint.

Surgery fol- lowed by a rehabilitation program or physical therapy with possible addition of an intra-articular corticosteroid injection are the two predominant treatments.

The majority of authors recognize that a well-designed rehabilitation protocol is essential to achieve good outcomes with both nonoperative and surgical treatment.

However, there is little evidence about what is the best rehabilitation protocol and most of the literature available is based on expert level opinion.

This current review investigates the recent literature on nonoperative and postoperative rehabilitation protocol and return to play in FAI patients and describes our approach.

  • (English) Episode 87-Overhead Athlete Shoulder Injuries - Decoding Diagnosis, Prevention and Treatment with Evidence-Base

This statement paper summarises and appraises the evidence on diagnosis, prevention, and treatment of common shoulder injuries in sports.

We systematically searched Medline and Embase. The Grading of Recommendations Assessment, Development and Evaluation tool was applied to evaluate the overall quality of evidence.

For diagnosis, we included 19 clinical tests from mixed populations.

Tests for anterior instability, biceps-labrum complex injuries and full subscapularis rupture had high diagnostic accuracy (low to moderate quality of evidence).

For prevention, the Oslo Sports Trauma Research Center, the Shoulder Control, the FIFA 11+ shoulder injury prevention programmes, and a baseball- specific programme (range of motion, stretching, dynamic stability and strengthening exercises) showed moderate to large effect size in reducing the risk of shoulder injury compared with no intervention (very low to moderate quality of evidence).

For treatment, a rehabilitation programme including stretching, ice packs, electrotherapy and compression, and strengthening exercises showed a large effect size in reducing pain and disability compared with no intervention in athletes with subacromial impingement syndrome (very low to moderate quality of evidence).

For the treatment of supraspinatus tendinopathy, hyperthermia treatment (heating the skin to 38°C–40°C) resulted in large effect size in reducing pain and disability compared with ultrasound or pendular swinging and stretching exercises (moderate quality of evidence).

Strengthening exercise alone or in combination with stretching exercises promoted a large effect in reducing shoulder pain (cohort studies, no comparators) (very low quality of evidence).

The quality of evidence for most estimates was low to moderate, indicating that future high-quality research may alter our recommendations for clinical practice.

  • (English) Episode 86-Neck Pain Relief - Global vs targeted exercises - the latest science

This document presents a randomized controlled trial published in the European Journal of Physical and Rehabilitation Medicine in February 2023.

The study, authored by Tânia Mendes Fernandes and colleagues, investigates the efficacy of "Global Postural Re-education" (GPR) compared to neck-specific exercises (SE).

The research aimed to evaluate the impact of these interventions on pain, disability, postural control, and neuromuscular characteristics in women experiencing chronic non-specific neck pain.

The findings indicate that both GPR and SE were equally effective in reducing pain and disability and improving neck mobility, though neither significantly affected postural sway.
