Recently, whilst running the IOSH Safety Management course, Colin sat back and reflected a little bit on accident investigation case studies discussed.
He noted there were 4 bullet points;
Colin thinks these are all fantastic things every business should be doing all the time, not only when there is an incident.
When there is an incident, there are a lot more people involved; management and even the senior management team join in. Colin wants to know, why aren’t we doing this already rather than only when there’s a problem, or worse a serious incident? Perhaps this could be avoided if we consider these 4 bullet points every day and on every job; not just as resolution to an incident
