They discuss how HSSIB works as an independent, no blame organisation, what the investigation found about the safety of Advice & Guidance and Single Point of Access, why national incident data may not fully reflect what GPs experience, and how practices can help by recording patient safety events through the Learn from Patient Safety Events (LFPSE) service.
