In this episode Vic and Jesse have a long overdue on air catch and talk through a human centred design project to improve medication safety in the Intensive Care Unit that Jesse has being working through in his day job. Vic and Jesse discuss, real world applications of Safety-II approaches, the core philosophy and practices of psychological safety and the importance of clinician led approaches to risk in practice.
