It happened in Kent, WA where a man's misfortune was caught on a gas station's surveillance cameras. First, a gun fell out of his pants. Second, he picked it up and tucked it back in his waistband. Third, he fell to the floor. Fourth, he got up and hobbled out. Long story short, he'd shot himself in the butt.
