The problem of over-treatment at the hands of aesthetic injectors runs deeper than just the aesthetic impact.
In this week's Aesthetic Mastery Show, Dr Tim digs into some important questions such as: is over-treatment subjective and should it just be the patient's prerogative what they have done to their face?
He also addresses the case of reality TV star, Molly-Mae Hague, who had an extreme jawline from dermal filler in 2019, and has since had it dissolved. Dr Tim explains why he believes treatments like Molly-Mae's and those from 'Kim K ' price-per-ML packages, could actually harm your business, as well as how to avoid the over-treatment trap.
You can hear Dr Tim speak at the virtual 'Overfilled Faces Diagnosis & Treatment' on Saturday 17th April conference. Book here: https://www.coreaesthetics.com/overfilled-faces-diagnosis-treatment/

Following last week's Aesthetics Mastery Show in which Dr Tim summarised the case of Julie Bass Kaplan FNP-BC who suffered a necrotic injury after she was treated with Radiesse, today we tackle the lessons to be taken from this important case.
Dr Tim analyses the various risk factors present, including the choice of a non-reversible filler; the injection site; the size of the bolus; the length of the needle; and the training environment in which the injections were carried out.
We examine the stages of necrosis that Julie experienced, and are lucky enough to learn from very well documented images.
Thank you once again to Julie for sharing her experience, and the invaluable learning for all of us that has come from that.

In 2010 Julie Bass Kaplan, an experienced nurse injector from California, was herself treated with Radiesse in the mid face during a training session. Julie suffered an infraorbital artery vascular occlusion, and went on to experience all the stages of necrosis.
When a clinician shares their own personal experience of a complication, as Julie did so graciously in this case, it is a true gift to the rest of the medical aesthetics community, because we get to learn from very well documented images and a detailed breakdown of each stage of the complication.
In this week's Aesthetics Mastery Show, Dr Tim goes into detail about the clinical aspects of Julie's case, including:
- Explaining what the mottled rash - livedo reticularis - is and how to look out for it in your patients.
- Some important learning about how the needle's angle of entry might have contributed to the occlusion.
- Which other vessels might have been affected and why.
- And a detailed breakdown of the stages of necrosis, including images from Julie.
Thank you so much again to Julie for sharing her experience, and the invaluable learning for all of us that has come from that 🙏
IN NEXT WEEK'S SHOW: Dr Tim will be sharing his analysis of the lessons clinicians can learn from Julie's case, so make sure you subscribe and turn on notifications for that.

Advanced Botox treatments in the lower face can be transformative for your patients but are also much more likely to cause side effects than upper-face toxin.
As Dr Tim explains in this week's Aesthetics Mastery Show, the reason is that lower third muscle anatomy is much more complex.
As well as a very helpful anatomy tutorial, Dr Tim also puts the spotlight on four advanced Botox treatments in particular: masseter reduction; downturned mouth; gummy smile; and platysmal bands, also known as Nefertiti lift.
He examines why particularly upsetting side effects from Botox are more common in these areas, and shares what aesthetic clinicians can do to avoid the worst happening.

Botulinum toxin treatments in the upper face are a real 'bread and butter' treatment in most aesthetic clinicians' practice, but are you familiar enough with the anatomy to avoid side effects, whilst also maximising product efficiency?
In this week's Aesthetics Mastery Show, Dr Tim takes us on an anatomical tour of the key muscles injected for upper-third Botox treatments, including the frontalis, corrugator, depressor supercilii, procerus, and orbicularis oculi.
He shares why you should avoid paint-by-number injection techniques; the anatomy you need to know in order to avoid eyelid ptoses; how to get procerus toxin doses right; key tips if you struggle to locate muscles in the first place; and the answer to the crucial question of whether you can treat the frontalis without also injecting the corrugator.
Dr Tim even shares a little-known tip to improve rhinoplasty results and longevity...using Botox.

Delayed onset nodules following dermal filler treatment - also known as lesions, or simply just lumps - can strike fear into the hearts of aesthetic clinicians because they are far from easy to diagnose and manage.
Firstly, you have to diagnose whether the lesions are just post-procedural, then whether they are inflammatory or non-inflammatory, then whether they're bacterial or not.
Should you watch and wait, and/or treat with antibiotics, steroids, and/or reverse the filler altogether with hyaluronidase.
In this episode of the Aesthetics Mastery Show, Dr Tim answers these questions and so much more about the diagnosis and treatment of delayed onset nodules.

In this week's Aesthetics Mastery show, Dr Tim explains why 80% of the ageing we see is due to the relationship between the ligaments and the fat pads, and how we can restore the impact using dermal fillers.
He gives us an anatomy lesson on the ligaments' function and location, and fascinatingly demonstrates using magnets and balloons the mechanical process behind why cheek filler lifts the jowls.

In this week's Aesthetics Mastery Show, we dive deep into facial anatomy to explore one of the most important facial structures for aesthetic clinicians, particularly in regards to injecting dermal filler: the deep fat pads.
Dr Tim gives us a detailed anatomy lesson on the upper, middle and lower third deep fat pads, including how to decide when to inject deep vs superficial fat pads.
We also discuss how to use the deep fat pads to build trust in the consultation and treatment design stages, including how to predict the number of mls of dermal filler you will need in certain areas.

Dr Tim returns to teach us about an anatomical feature that is central to successful aesthetic treatments because of their impact on the shape of the face; the harmony between structures; and its proportions. In this week's Aesthetics Mastery Show, we tackle the superficial fat pads.
We discuss fat pad depletion with age; the vital impact that fat pads have on the consultation process and educating our patients; and how we should use them to design injectable treatments.

For the next four episodes of the Aesthetics Mastery Show we have a special treat - some never-seen-before footage of a sold-out conference we hosted last year, Dr Tim Live.
The event was designed to walk delegates through what it's like to undertake a whole-face 14ml treatment from start to finish.
In the first of four excerpts, Dr Tim marks up his model, Lindsay, and shares his '5 Q Algorithm' - the 5 questions you must ask yourself before you inject safely, and in order to produce beautiful results.
