The Caldwell-Luc procedure is often selected to perform a sinus lift (also known as a maxillary sinus augmentation) to add bone to the posterior maxilla for future dental implant placement. This modified approach involves accessing the maxillary sinus through the lateral wall of the maxilla to elevate the sinus membrane and graft bone material.

Steps:

1. Incision:

• A buccal incision is made in the vestibule in the posterior maxilla.

• The flap is reflected to expose the lateral wall of the maxilla, directly over the sinus.

2. Creating the Window:

• A bony window is created in the lateral wall of the maxilla using a burr or piezoelectric device. Care is taken to avoid perforating the sinus membrane (Schneiderian membrane).

3. Sinus Membrane Elevation:

• The Schneiderian membrane is gently elevated from the floor of the sinus using specialized instruments, creating a space for the bone graft material.

4. Bone Graft Placement:

• Bone graft material is packed into the space between the sinus membrane and the bony floor of the sinus.

5. Closure:

• The mucoperiosteal flap is repositioned and sutured, ensuring a tension-free closure.

• If membrane perforation is encountered, a membrane is commonly utilized to protect the membrane and separate from the bone graft.

Indications:

• Insufficient bone height in the posterior maxilla due to:

• Sinus pneumatization (natural expansion of the sinus with age or tooth loss).

• Alveolar bone resorption after tooth extraction.

• Preparation for the placement of dental implants in the posterior maxilla.

Advantages:

• Provides sufficient bone height for implant stability.

• Facilitates implant placement in areas where sinus pneumatization would otherwise preclude it.

Complications:

• Sinus membrane perforation: The most common complication, which can compromise the graft or lead to infection.

• Postoperative sinus infection: Sinusitis may occur if the sinus becomes contaminated.

• Graft failure: Inadequate healing or bone resorption may occur.

Comparison with Crestal Approach Sinus Lift:

• The Caldwell-Luc lateral wall approach is used when a significant bone height increase is needed (e.g., more than 4-5 mm).

• A crestal approach is minimally invasive and typically used when minor augmentation is sufficient (e.g., less than 4 mm)
