Generating scientific evidence is, although essential, just the first step. For clinical practice to be driven by evidence we must ensure that the knowledge is translated into policies that guide practice and are available for all. But how? The road from research to clinics is not a linear one, nor it is the same in every setting. Trials conducted in high-income countries can offer limited support to practice-changing in other contexts.
