The company behind the Oreo has announced back in May it is considering a CBD-infused cookie or food item to be added to the plethora of snacks available on the market. Mondelez — which makes Oreo, Cadbury, Chips Ahoy, Nutter Butter and Ritz — revealed plans to hopefully launch a snack infused with CBD as interest continues to grow in the market for the food products. Meantime, the FDA has continued to issue warning letters to companies selling CBD-derived human and animal products. "Warning Letters" are administrative actions that are akin to cease and desist letters. The majority cited hemp or CBD-derived human ingestible products, such as oils, tinctures, gummies, lollipops, and capsules. FDA's most recent warning letter was published in September 2019.   • Cannabis Drug Interactions and Side-effects, with Dr. Franjo Grotenhermen
    In this interview I spoke with Dr. Franjo Grotenhermen who is Executive Director of the International Association for Cannabinoid Medicines and organizer of the 2019 Cannabinoids in Medicine conference in Berlin, Germany.

    Dr. Grotenhermen treats many patients using high-THC cannabis, including over 300 with ADHD and some with schizophrenia. At the conference he shared his experience and expertise with doctors from 35 countries, and he kindly shared shared some with us.

    Dr. Grotenhemen explained potential drug interactions. He also addressed possible acute side-effects like psychosis, and possible chronic side-effects like dependency and schizophrenia. He explained these are insignificant when put in perspective.

    He also shared research that indicates many medical conditions may originate in the endocannabinoid system (which cannabis helps regulate), and research into THC as an anticancer agent.

    Cannabis for Tourette Syndrome alone and with comorbid ADHD, with Prof. Dr. Kirsten Mueller-Vahl. We spoke with psychiatrist and neurologist Prof. Dr. Kirsten Mueller-Vahl at the 2019 IACM Cannabinoids in Medicine conference in Berlin, Germany. Prof. Dr. Mueller-Vahl outlined the progress toward large-scale clinical trials that can demonstrate the efficacy of cannabinoid medicines for Tourette Syndrome and common comorbidities like ADHD. Prof. Dr. Mueller-Vahl explained the hope of developing a treatment that addresses both Tourette and comorbid ADHD through the endocannabinoid system. This single treatment option would reduce the likelihood of adverse events stemming from interactions between the concurrent treatment strategies currently used (which are dopamine modulation in opposite directions). She notes that evidence suggests cannabinoid medicines may treat the root cause of these conditions, rather than just treating symptoms, however, more research is needed. She explains that such studies currently have difficulty obtaining funding. Prof. Dr. Mueller-Vahl recommends doctors currently consider prescribing cannabis for adult patients who do not benefit from standard treatments. This is despite the current lack of efficacy trial data. She also notes that when the clinical database is strong for cannabis in adults with Tourette, children should also be studied to improve symptoms and possibly influence the cause of the disease. Prof. Dr. Mueller-Vahl states that in adults with no history of psychosis there is no risk of developing schizophrenia from using cannabis medicines. She says a psychotic episode may be induced but this is merely a side-effect of intoxication. She agreed this is similar to being drunk on alcohol.

  • Where We Are And Where We're Going, with Ethan Russo MD
    We spoke with US-based neurologist Ethan Russo MD, Director of Research and Development at the International Cannabis and Cannabinoids Institute, at the 2019 IACM Cannabinoids in Medicine conference in Berlin, Germany. Dr. Russo outlined challenges for conducting clinical trials of cannabis for the various medical conditions that are already supported by subtantial evidence warranting further studies. He explained the problem of getting funding and approvals for clinical trials. Dr Russo described the impact of increased placebo-responses over the past decades, and that subjects may see through blinding if they feel intoxicated, but notes the existence of a sweet-spot where symptoms can be treated without intoxication. The argument that cannabis use may increase the likelihood of triggering psychosis or schizophrenia in patients is also addressed. Dr Russo said the incidence of schizophrenia in society had dropped despite a significant increase in recreational cannabis use. He also notes that there is no impairment in patients using normal doses and that cannabinoid medicines can be prepared that do not intoxicate, but that precautions should be taken. Dr Russo points out that in two recent Phase II clinical trials CBD has been shown to treat schizophrenia with far fewer side effects than traditional antipsychotic medicines.

    Cannabis for ADHD, with Aleksi Hupli. Researcher Aleksi Hupli explored cannabis for ADHD in his recent case report Medical Cannabis for Adult Attention Deficit Hyperactivity Disorder: Sociological Patient Case Report of Cannabinoid Therapeutics in Finland. Aleksi talked to us about his study, including discussion of some of the most significant evidence to date supporting the use of cannabis for ADHD. He also mentions a follow-up study he'd like to conduct, to explore the topic more thoroughly. Aleksi lives in Amsterdam, from Lappeenranta, studied in Helsinki and currently doing my PhD at the University of Tampere. He also works as a freelance video reporter and is a board member at the Finnish Association for Humane Drug Policy and the vice-president of the Amsterdam Society for Medical Social Scientists.

    The plight of ADHD and Depression patients seeking medical cannabis in Germany, with Maximillian Plenert. We spoke with speaker of the German cannabis patients' association Maximillian Plenert. He explained that recent law changes have allowed for health insurance coverage of cannabis medicines, but that companies dont want to cover them for conditions like ADHD or depression. He says 1 gram of cannabis from a pharmacy costs more than 20 Euro (22 USD, 32 AUD), so without insurance coverage many patients turn back to the black market. Adding to the challenges faces by patients, many doctors don't want to prescribe cannabis for these conditions. Maximilian also runs a patients hotline that is constantly taking calls from frustrated ADHD patients who can't get access to or afford the cannabis medicines that help them to function well. He says ADHD patients are the largest homogenous group of patients and suggests the possibility of bringing many of together to "crash into the system" and raise awareness of their terrible struggles to access and/or afford cannabis medicines.

    Cannabis for Children, a Pediatrician's Experience, with Dr. Bonni Goldstein. We spoke with Dr. Bonni Goldstein at the IACM Cannabinoids in Medicine conference 2019 in Berlin, Germany. Dr. Goldstein is a trained pediatrician and practicing cannabis specialist in Los Angeles, USA. Dr. Goldstein has successfully treated over 900 children with serious medical conditions including epilepsy, autism, various cancers, anxiety, depression, bipolar, eating disorders, OCD and ADHD among others, using cannabis medicines. Dr. Goldstein explained to her "do no harm" approach to giving these otherwise untreatable kids and their families a second chance to live healthy, happy lives. She said patients achieve the quality of life improvements, fewer side-effects, more alert, more awake, starting to talk, using fewer pharmaceuticals, fewer hospitalizations, and other benefits. Dr. Goldstein also said other physicians are now understanding that they can alleviate unnecessary suffering using cannabis as medicine. She describes that for pediatric epilepsy this means significant reductions in potentially brain-damaging seizures or even complete freedom from seizures [without the terrible side effects of current anti-epileptic drugs]. In her practice, cannabis medicines reduce the crazy behavior of kids with autism to reveal the person underneath [where there is no safe and effective treatment currently available].
