  • The Hardest Truths About Scaling a Business | Operator Standard Q&A - EP312

I got on an open forum call with a room full of operators and told them to ask me anything, business, hiring, scaling, partners, family, all of it. No script, no filter, just real questions from people actually building. This is the stuff that doesn't make it into a polished keynote.

We covered a lot of hard truths in this one. The first one I hit them with up front: to get where you want to go you're going to have to sacrifice more than you think is enough, for probably twice as long as you think you'll have to. Most people quit right in the middle of that stretch. I also got into why, if I could scale again, I'd go slower. Growing too fast meant hiring a pile of bodies and then turning around and firing them and getting back to our core values meant replacing most of the team to rebuild the standard we started with. What got you here won't get you there.

I broke down the mistake of expanding into too many cities and states at once and why you should own your zip code first, become the go to business in your city, then spread one zip code at a time instead of jumping four cities over. We talked about when to actually take on an investor and the trap of giving up equity to someone who only brings money when a bank would give you that same money without taking a piece of your company. If a partner brings money and the expertise and the life you actually want, that's a different conversation and I walked through how to structure equity, voting rights and buy-back so you never end up a minority in your own business.

Then we got into leadership and people. Why you build systems and processes that carry your core values instead of holding the whole thing together with your own hands. Why keeping a talented person who doesn't fit your standards lets the problem spread through the whole company. And a mindset that changed how I lead, everybody around you is making decisions because they think that decision will make them happier and understanding that gives you grace as a leader without lowering your standard.

If you're an operator, a founder, or a tradesperson trying to build something real, this is a long, honest Q&A full of the things I wish somebody had told me earlier.

  • I'm Old Enough to Buy Eggs (Stop Shrinking Yourself to Fit In) - EP 311

Every Halloween growing up, my dad didn't just take us trick or treating. He dressed up too, in full costumes he hand sewed himself. And this grown man would walk right up to the door next to us and say trick or treat. People would look him up and down and go aren't you a little old for this? And every time he'd look them dead in the eye and say well, I'm also old enough to buy eggs now. Give me the candy or I'll be back later.

I thought he was just being goofy. It took me years to see what I was really watching. A man being completely, unapologetically himself in a world that kept telling him he was too old, too big, too much. And he did not care even a little.

This episode is about that voice in your head, the one that shows up the second you want to say the thing, build the thing, become the thing and whispers act your age, that's not professional, what will people think. That voice is not you. It's a committee. Every teacher who told you to sit down, every boss who wanted you smaller, every person who was more comfortable when you were a watered down version of yourself. You've listened to it so long you think it's your own voice. It's not.

Most people spend their whole lives being who they think they're supposed to be. They pick the safe major, the respectable job, they sand off every edge that makes them interesting until they're smooth and forgettable and then one day they're fifty wondering why their life feels like it belongs to someone else. I'm a doctor who refused to be what a doctor is supposed to be and here's what I've learned. The loudest voice telling you to shrink usually comes from someone who already shrunk themselves, and it kills them a little to watch you refuse to do the same.

That thing you keep talking yourself out of is usually the most valuable thing you've got because it's the one thing nobody can copy. Anyone can be the acceptable version. There are ten thousand of those. There's exactly one of you being you. You're going to get old either way. The only question is whether you spend those years being yourself or being a costume the committee approved.

CHAPTERS 00:00 My dad went trick or treating in full costume 01:15 I'm old enough to buy eggs 01:56 What I was actually watching all those years 02:20 The voice in your head is a committee, not you 03:16 How people shrink until their life isn't theirs 03:51 The doctor who refused to stay in the lane 04:37 The people telling you to shrink already shrunk 05:09 The thing you talk yourself out of is your biggest asset 06:41 Kill the committee and be yourself on purpose

  • How to Structure, Scale and Protect a Business From Scratch | Operator Standard Q&A - EP 310

I sat down with a room full of operators from East Tennessee and told them to throw every business question they had at me. No script, no filter. Just real questions from people actually building and the same answers I'd give if we were sitting across the table.

We covered a lot of ground in this one. When it actually makes sense to acquire another business versus pouring that money into what you already have and the expensive mistake I made expanding into too many territories before I dominated the one I was in. If I could go back, I'd build ten locations and make each one the go to place in its city, instead of spreading myself across eighty and watching them all spin their wheels. Own your zip code first, then move to the next one.

I got into how I actually structure my companies. Separate LLCs for each business so nothing cross-contaminates in a lawsuit, a management services agreement running up to a Delaware based mother ship, and management fees flowing out to personal LLCs so there are layers of protection around your income. I'm not your attorney or your CPA and you need to have that conversation with your own tax person, but I walked them through exactly how mine is built and why.

We talked vertical integration, how to look at your P&L, find your highest cost of goods and decide whether to negotiate it down, partner strategically or acquire a piece of the supplier so you're getting your product at cost. I explained why I'd avoid taking out loans at almost any cost when you're starting, why learning to run a business with no money is what makes you resourceful and why handing a broke operator a pile of cash usually ends the same way it ends for lottery winners. And I shared the note taking system I actually used to build everything, the part nobody does, which is going back and applying one lesson at a time instead of just collecting notes you never use.

If you're an operator, a founder, or someone trying to build something real from scratch, this is ninety minutes of the stuff I usually only say behind closed doors.

  • Your ADHD Isn't a Disorder. It's a Ferrari Engine With No Track - EP 309

When I was a kid I got diagnosed with ADD. Back then they called it something else but the story was the same. I couldn't sit still. I couldn't shut it off. My brain was going a hundred miles an hour in every direction at once, all day, every day. So the school did what schools do. They looked at my parents and said we think you should put him on medication, calm him down, get him to sit quietly in a chair like the other kids.

    My parents said absolutely not.

My dad was blue collar, Metallica in the garage, Craftsman toolbox, figure-it-out kind of guy. My mom was Puerto Rican, salsa and movement in the house all the time. These were not soft people. And when the school said let's medicate your son so he's easier to manage, my dad basically said you're not sedating my kid so he's more convenient for you. We'll handle it. And here's how they handled it. They didn't try to make me calmer. They gave me somewhere to put it. At one point I was in five different sports at the same time. They kept me moving, they kept me challenged and my dad's whole thing was school is your job, do it well, then go burn the rest of that energy off on the field.

They didn't treat my intensity like a problem to dial down. They treated it like fuel that needed somewhere to go.

Here's what I've come to believe after living this myself. And let me be clear, because I'm a doctor. Some people genuinely need medication. That's a real medical decision made with a real physician and I'm not touching that. I'm talking to the other group, the big group, the ones using the diagnosis as an excuse for why they can't be successful. For a lot of you it was never a broken brain. It was a Ferrari engine sitting in a school desk in first gear, redlining, going nowhere. The problem was never too much energy. The problem was nobody gave that energy a track to run on.

This episode is about ADHD, intensity, and why the exact wiring people keep telling you to calm down is the thing that builds companies. Every person I know who has built something real is a little bit wired wrong by normal standards. The calm, easy-to-sit-next-to people are wonderful and they're usually working for the intense ones. But here's the catch. An engine with no track destroys itself. If you don't give that energy an outlet it doesn't disappear, it turns on you. It becomes anxiety, ten unfinished projects, lying awake at two in the morning with your brain screaming at you. The energy is coming out either way. The only question is whether you build the track or it tears up the whole car.

So stop trying to become the calmer person. You're not going to become the calmer person. Build the outlet instead. Structure means systems and real processes so your brain isn't the only thing holding the business together. It means hiring the calm, detail-obsessed operator who does the thing you'll never do, so you can run down field where you're actually valuable. And you still need the physical outlet, the gym, the sport, the place to dump the excess so it doesn't leak into your marriage or your team.

Stop treating your wiring like a diagnosis you're stuck with. Stop letting a label become the ceiling you build your whole life under. If you genuinely need help, go get it, no shame in that. But if you've got a powerful engine and no track, quit trying to sedate it, quit apologizing for it, and go build something big enough to hold all of it.

They didn't turn me down. They pointed me at the field and let me go.

CHAPTERS 00:00 The decision my parents made when I was a kid 00:17 The school wanted to medicate me 00:54 Who my parents were and why they said no 01:26 They didn't make me calmer, they gave me somewhere to put it 02:03 The part I have to say as a doctor 02:39 A Ferrari engine stuck in first gear 03:08 You don't fix a powerful engine by choking it 03:29 To the entrepreneur who can't turn it off 03:58 Your intensity is the asset, not the defect 04:22 An engine with no track destroys itself 05:01 Stop trying to be calmer, build the outlet 05:45 The life I'd have if my parents said yes 06:15 Stop letting a label become your ceiling

  • The Warm Body Trap That's Quietly Killing Your Clinic - EP 308

Picture the Trojans, ten years into the war. Ten years of dead sons, food running low, hope running lower. Then one morning the Greek camp is gone, the ships are gone and standing at the gates is a giant wooden horse. One man, a priest named Laocoon, throws a spear at it and screams don't bring that thing inside our walls. They ignored him. They were too tired to be careful and that one bad night at the gates cost them everything they had built.

That is exactly how most clinic owners hire.

In this episode I break down the warm body trap, the hiring mistake that has destroyed more clinics than slow months, bad rent and crappy marketing combined. A warm body is anyone you hire because they have a pulse and credentials, not because they fit your character and your standards. And owners don't hire them because they're dumb. They hire them because they're exhausted and that horse in the moment represents the one thing they want more than anything, which is relief.

I get into the real math of a bad hire and it's uglier than owners think. That 120 grand nurse practitioner doesn't cost you 120 grand when she's wrong. She costs you somewhere between 180 and 360 grand once you count the salary you keep paying while she fails, the revenue a good person would have produced, the damage she does to your best people, the patients she quietly runs off and the cost of starting the whole search over from a worse position. I walk through a real story of an owner who hired in pain, watched an NP build a personal patient base for twelve months, then lost half a million dollars when that NP walked out the door and opened up down the street.

Then I get into the alternative, how to hire the right way so you stop bleeding. Always be recruiting, even when you don't have an open seat. Hire on character and core values, not credentials. Evaluate with stories, not claims. And be the most picky exactly when you are the most desperate. Because every person you let in the door is a vote for the kind of business you're becoming. Let in warm bodies and you become a warm body clinic. Let in the right people and you become a clinic of right people.

If you own a med spa, aesthetics practice, chiropractic, dental or any clinic where your staff is the product, this one will save you a hire you're about to regret.

  • The One Broken Detail That Defines Your Whole Business - EP 307

I was driving with a buddy a while back and we pulled up next to a Nissan Versa. Out of nowhere he goes, how much you want to bet the rear passenger hubcap is off? I said what are you talking about. He goes, just watch. We get closer and sure enough, rear passenger side hubcap, gone.

Turns out his wife used to drive a Versa and that one hubcap would never stay on. So they started noticing them everywhere. Highway, neighborhood, all over town. Versa after Versa, different owners, different tires, same rear passenger side, same missing hubcap. Here's this otherwise solid little car, good on gas, runs fine, does everything it's supposed to do and it's rolling around looking cheap because of one missing piece.

I've been chewing on that ever since. Because that hubcap is the difference between average people and excellent people. Average people look at the car and go, who cares about a hubcap, it runs fine. Excellent people look at the same car and go, I don't care how good everything else is, I'm not driving around with the hubcap falling off. Fix it.

And I'll be honest with you, I've had my own hubcap. When we were scaling my company, growing fast, new locations opening, revenue climbing, it's easy to fall in love with the big stuff and let the small stuff slide. The way phones got answered at one location versus another. The way a patient got greeted walking through the door. I told myself the same lie every owner tells themselves. Everything else is good, it's just a hubcap, who cares.

But a patient doesn't experience your revenue. They don't experience your growth chart. They experience the front desk. The first ninety seconds. I read a review one day, five stars for the provider, and then this line: great results, but the front desk made me feel like a number. That one stuck with me. The medicine, the hard thing, we nailed it. The thing that should have been easy, the hello, the follow up, that's what they remembered and told their friends about.

This episode is about excellence, standards and why the boring unglamorous details are the ones your patients and customers actually feel. Average operators protect the parts everybody claps for. Excellent operators hunt down the hubcap nobody's talking about, because there's no glory in it. Nobody throws a party because you finally answered the phones on the second ring. But that boring stuff is the whole story people tell about you.

You've got a hubcap. I promise you've got a hubcap. Maybe your treatments are world class but your front desk lets calls go to voicemail. Maybe your results are incredible but your follow up is a mess. Maybe your building is beautiful but your billing is a nightmare. Your patients don't see the engine. They see the missing hubcap, and that one broken detail becomes the whole story they tell about you.

So here's your homework. Look at your business like you're the guy sitting in the passenger seat. Find the one thing you've been winking at. Stop winking at it and fix it. Then go find the next one. Because excellence isn't one big heroic moment. It's a thousand hubcaps you refuse to tolerate.

  • You Become What You Tolerate in Yourself - EP 306

It's 11:14 at night. You're in bed, phone in your hand. You told yourself two hours ago you'd be asleep by 10:30. You're not. You scroll one more reel, then one more. You check email even though nothing's new.

Nobody sees this moment but you. Your wife is asleep next to you. Your kids are down the hall. Your team is offline. The only person watching what you do right now in the dark is you. And that is where the rot starts.

You've heard me say it before. You become what you tolerate. Most of the time when I say that I'm talking about your business, your team, the leader who treats people like garbage, the manager who keeps missing the number. Today I want to talk about the harder version. The one nobody can fix for you. What you tolerate in yourself.

The thing that takes great operators out is almost never the market. It's not the competitor down the street. It's not the recession. It's the slow personal drift. The second drink that becomes three. The workout you skipped Monday that turned into a skipped February. The conversation you keep avoiding. The "I'll start Monday" you've said for fourteen Mondays.

Here's what's brutal about it. Nobody else sees it. Your team sees a leader. Your customers see someone who has it together. Your wife sees a tired husband working hard. But you see the gap between who you say you are and what you actually did when nobody was looking. That gap is everything and the longer you carry it the heavier it gets.

This one is about self-discipline, self-sabotage and the quiet daily habits that build or destroy high performers over time. I get into why the same skill that runs your business also runs your personal drift, why nothing that happens to you is ever really sudden, it's just suddenly visible and the one small thing I want you to do this week to start closing the gap. Not five non-negotiables. Not a ninety day transformation. One thing you don't break, because identity isn't built on big promises. It's built one rep at a time.

If you're sitting in your truck or at your desk right now thinking he's talking about me, this episode is for you.

  • My Mom Has Dementia, What It's Teaching Me About Life - EP 305

With dementia you don't lose someone all at once. You lose them a little bit at a time.

Some days my mom knows exactly who I am. Some days I'm just a kind face sitting next to her. And that has forced me to sit with a kind of grief most people don't talk about, the grief that shows up while the person you love is still here.

In this episode I get personal about what my mom's dementia has taught me about loss, gratitude and how we choose to live while we're still losing someone. I've watched people handle grief in every possible way. Some go quiet and withdraw. Some stay busy so they never have to stand still. I've done both. And somewhere in the middle of it I stopped asking whether I was grieving the right way and started asking a better question. Am I honoring her with the way I'm choosing to live?

Because here's what I keep coming back to. The people who truly love us don't want our grief to stop our lives. They want their lives to continue through ours. The greatest tribute you can pay someone isn't the amount of sadness you carry after they're gone. It's how much of their life still lives through yours.

My mom taught me how to show up. How to be present. How to make everyone who walks through the door feel like they matter. I built companies with hundreds of people in them, and when I ask myself where I actually learned to care about people, it wasn't a business book or a seminar. It was her kitchen table.

This one is for anyone walking through loss right now, or watching a parent slip away, or bracing for grief they know is coming. Grief is real. Loss is real. But eventually grief has to become gratitude and sadness has to become purpose. And maybe a world-class life isn't about avoiding pain. Maybe it's about refusing to let grief have the final word.

Love doesn't require recognition. I don't show up so she'll remember me. I show up because I remember her.

  • The Membership Mistakes Costing Your Clinic Six Figures a Year - EP 304

Can you say out loud right now exactly how much money is going to hit your account next month? Not a guess. Not "it depends on how busy we are." An actual number.

Most clinic owners can't. And that one blind spot is quietly costing them six figures a year.

I had a call in my medical mastermind with a clinic owner who had been open almost four years. Good clinician, steady patients, decent reviews, busy schedule. When I asked her what was hitting her account next month, she laughed. Not a nervous laugh, a real one, like the question was absurd. Four years in business and she had no idea. She was working just as hard in year four as she did in year one because every single month she was starting from zero again, hunting for the next round of patients to replace the ones who came and went the month before.

That is not a business. That is a treadmill that hopefully produces cash. And you cannot outwork it because the problem is not effort. It is structure.

The fix is recurring revenue. Memberships. Patients on auto billing, getting a service on a schedule, month after month, without you having to resell them every single time. But most clinic owners get memberships wrong before they ever touch a number and they burn months building something that was broken before it launched.

In this episode I walk through the eleven mistakes that quietly kill med spa and clinic memberships, why most owners get the model wrong before they touch a single number and how recurring revenue does not just smooth out your cash flow, it changes what your entire business is worth when you go to sell it. Buyers pay a premium for predictability and a clinic that knows what is coming next month is a completely different asset than one starting from scratch every thirty days.

If you run a med spa, aesthetics practice, wellness clinic or any medical business built on repeat patients, this one is about building the recurring revenue and membership model that finally lets you stop starting every month at zero.

  • The New FDA Peptide Rules Every Med Spa and Clinic Owner Needs to Know

The FDA just reclassified peptides and it is going to reshape how med spas and medical practices operate in 2026.

In this episode I break down exactly what the new FDA 503A peptide rules mean for your med spa business, which compounds are still legal to prescribe and the four step framework I am using to protect my own clinic and actually grow from this instead of getting crushed by it.

Most clinic owners are going to panic or ignore this. I have watched both play out and neither one works. I walk you through what goes in the trash, what you can do this week with zero legal risk, what to build over the next few months and the long game I am playing while everyone else waits to see what happens.

I also go compound by compound through the peptides every physician entrepreneur should understand right now, from BPC 157 and TB 500 to Semax, MOTS-c, GHK-Cu, and more, and where each one fits inside a real med spa business plan, a GLP-1 program or a recurring revenue membership model.

If you are a physician entrepreneur, med spa owner or healthcare professional trying to move from doctor to CEO, this is the exact conversation I wish someone had walked me through before I built 80 locations.
