讲完"风湿性关节炎" rheumatoid arthritis 的基本定义后，这期我们就来细数RA的临床症状吧！

Rheumatoid arthritis has an insidious onset of weeks to months.

Insidious 这个单词很有意思，在医学语境中指"不知不觉就恶化的"，与 gradual onset 相近。但在日常用语中，则是形容某人"阴险狡诈"，属于贬义词。

Rheumatoid arthritis usually affects the same joints on both sides of the body. The joints symptoms include pain, stiffness, redness, warmth to the touch and joint swelling.

上文中的第一句话也可以缩略成 symmetrical joint involvement 以表示RA大多影响对称关节。

第二句中的 pain, redness, warmth and swelling 并称为 core features of inflammation。

The joint stiffness is most bothersome in the morning and can persist for more than an hour.

这句话所描述的就是风湿性关节炎最有特征的"晨僵" morning stiffness 了。

因为阑尾炎最常见的腹痛位置在右下腹一个叫作 McBurney’s point 的地方。

McBurney’s point：A point that lies one-third of the distance laterally on a line drawn from the umbilicus to the right anterior superior iliac spine.

相关词汇:

●Laterally "外侧的"

●Anterior superior iliac spine "骼前上棘" (缩写ASIS)

*但腹痛位置也会因阑尾的先天位置不同而变化

Location of pain may vary depending on the position of the appendix which leads to atypical presentation of appendicitis.

2.胆管炎最具有代表性的临床表现，就是"Charcot 三联征" Charcot’s Triad 了

(2).RUQ pain "右上腹痛"

-RUQ 为 Right Upper Quadrant 的缩写。Quadrant 的本意是指数学中的"象限"，在报告查体结果时较简略的方法就是将腹部一分...

1.Cholecystitis 这个单词可以分解成三部分：

Chole-cyst-itis

(1).前缀 chole- 代表与"胆道系统" biliary system 相关的,

例如我们刚开始几期提到的"胆固醇" cholesterol。

(2).Cyst 本意是"囊; 泡"的意思，例如 ovarian cyst "卵巢囊肿"。

(3).我们在介绍"关节炎" arthritis 的时候有讲过，-itis 代表"炎症"。

这个后缀在医学英语中出现频率相当高，大家请重点记下！

2.然而 Cholecystitis 的通俗说法是 Inflammation of the Gall Bladder

(1).Gall 和 Chole- 意思相同，只是 Gall 的用法更通俗。

例如非医学背景的大众通常称"胆结石"为 gallstone。

(2).然而书面医学英语的说法则分得更细:

●"胆囊结石" cholelithiasis

●"胆管结石" choledocholithiasis

●-docho- 代表"管道" (duct)

3.有意思的是，bladder 单独使用代表"膀胱"，而 gall bladder 却是"胆囊"。

(2).Herceptin (Trastuzumab) "赫赛汀"

它用来治疗"HER2+乳腺癌" HER2+ breast cancer。

(3).Chemotherapy "化疗"

(4).Radiotherapy "放疗"

1).“骨关节炎” Osteoarthritis

1.Joint space narrowing "关节空隙减少"

2.Osteophyte 意为"骨质增生"。

⦁前缀 osteo- 之前讲过代表"骨骼的"。

⦁-phyte 这个后缀原本是"植物/植物般"的意思，因为"骨质增生"就像植物生长一样因此就有了 osteophyte 这个说法。

3.在关节边缘还有一些硬化的改变。

"There are also some sclerotic changes at joint margins."

⦁Sclerotic 指"硬化的"，名词形式 sclerosis 指"硬化的过程"。像"多发性硬化"就是 multiple sclerosis。

⦁Margin 则是"边缘"的意思，像用Word文档时要调整 margin size "页边距"。

⦁比较"边缘化的人群"也可以称为 marginalised population。

这期我们要聊的是和心血管疾病 cardiovascular disease 仅有"一字"之差的脑血管疾病 cerebrovascular disease~

脑血管疾病中最具代表性的就是脑卒中 cerebrovascular accident (CVA)了。患者通常称之为stroke中风, 但因为CVA全称比较拗口, 医护之间也常stroke。Cerebrovascular accident 则基本只出现在书面医学英语。我们今天就来说说不同类型的脑卒中 (different types of CVA) ~

1.Ischaemic stroke "缺血性卒中"

[ischaemic/ischaemia] "组织中供血不足"的定义为——

"A restriction in blood supply to tissue."

2.Haemorrhagic stroke "出血性卒中"

其中, haemorrhagic的名词形式为 haemorrhage,其前缀haem- 代表"血液"。

衍生词汇:

"血液科" haematology

"血液科医生" haematologist

3.Transient ischaemic attack "短暂性脑缺血发作"(缩写TIA), 通常症状持续时间不少过24小时:

"Symptoms fully resolved within24 hours in the case of TIA."
