Dr. Treadway explains the many kinds of testicle pain and how they can be linked to over or under exercising. Often old injuries are aggravated by changes in lifestyle such as a new exercise regimen or a change to more sedentary work. Sitting in a stooped posture can worsen existing pain by shifting weight onto your tailbone, causing misalignment in the spine and over-tightening of the pelvic floor muscles. In an attempt to combat this problem, the body often "tucks" the tailbone much like a puppy tucks his tail when stressed. Fortunately, Dr. Treadway has decades of experience relieving all manner of pelvic floor problems, particularly through the use of biofeedback sensors that help patients understand how to lengthen and strengthen those muscles. In this segment, Dr. Mistry and Donna Lee answer a listener's question on treatment for Peyronie's disease. If you have developed a bend in your penis or if, when erect, it stands at an unnatural angle, you may have Peyronie's disease. While the exact cause is unknown, Peyronie's disease may be caused by the slowly accumulating effects of microtrauma to the penis sustained during intercourse with a less-than-fully-erect penis. Scar tissue can develop along the penis shaft, causing the often painful and distressing changes associated with this incredibly common condition. More severe, acute trauma to the penis can also cause Peyronie's to develop practically overnight,

Fortunately, there are many treatments for Peyronie's disease that address its various causes and effects. In this episode, Dr. Mistry explains some of the different treatments for Peyronie's. Injections of Xiaflex directly into the penile plaques (which break them down) have about a 70% success rate in reducing the penile curvature. If you've tried Xiaflex and it hasn't worked, surgical intervention is needed. There are two main types of surgery for Peyronie's disease.  In plaque excision and grafting surgery, they remove the hard plaque that's causing the curvature and replace it with a better, more pliable substance. In placation surgery, stitches are placed on the non-curved side of the penis to straighten the curve out. These two options have different costs/benefits to consider, but both are commonly performed by our expert surgeons and can make a real, lasting difference in your sex life! No matter your age or disease progression, getting an evaluation and proper measurement of your curvature by a urologist is essential in identifying the right treatment. In this segment, Dr. Mistry and Donna Lee give more information about treatments for urinary incontinence, which can be life altering in its consequences. We often talk about our wonderful pelvic floor physical therapy options for people struggling with urinary leakage, and this treatment is helpful for incontinence as well as a number of other urinary complaints. Today, Dr. Mistry and Donna Lee are excited to discuss a newer treatment option, the Axonics neuromodulator, which uses electrical stimulation to send signals that encourage your bladder nerves to behave themselves. We carefully screen the patients for whom we recommend the initial trial of this device, so the majority go on to get the device fully implanted along with a small battery that is MRI-safe (unlike many older neuromodulating devices). Another option is the injection of Botox into the bladder, which helps calm spasms and misbehaving nerves. These procedures are usually covered by insurance when the patient tried prescription medications but couldn't tolerate their side effects (which often include dry mouth and constipation or diarrhea and high blood pressure, depending on the medicine). In this segment, Dr. Mistry and Donna Lee discuss options for men looking to beautify their penises. Among our male patients and listeners, interest in plastic surgeries for penises is increasingly common. While no surgery can ACTUALLY increase the length of your penis, there are several procedures that give the appearance of a thicker, longer penis and enhance its look and feel. For men with concerns about the aesthetic appeal of their penis, there are many options to treat or repair the penile skin. We frequently use a filler gel to create volume much like women use fillers to do the same in cosmetic facial procedures. The fillers typically last several years and produce a 1-2 centimeter increase in penile thickness while also enhancing sensation. Vacuum erection devices can help straighten the penis and therefore contribute to an increased length of 0.5-1 centimeter with regular use. For patients suffering from "buried penis," we can reducing the size of the fat pad above the penis (which "buries" it) through liposuction or flap surgery to tuck away excess skin. We also perform what's called the "Lazarus Surgery" for severely buried penises which, as the name suggests, can raise them from the dead, so to speak. Finally, the removal of scrotol webbing can help men whose penises are restricted due to botched circumcisions or similar problems. Most of these surgeries cost between $4000-$6000, less for fillers. In this segment, Dr. Mistry and Donna Lee answer a listener question about whether HIFU--high intensity focused ultrasound--can be used to reduce the size of an enlarged prostate. The HIFU procedure is a state-of-the-art treatment for prostate cancer. It selectively targets cancerous areas of the prostate and then ablates only those areas with minimal damage to the surrounding tissue. This is an amazing advance in treatment because other prostate cancer treatments can sometimes lead to urinary incontinence and/or erectile dysfunction. However, Dr. Mistry explains that HIFU is not used to reduce the size of the prostate. He recommends a non-invasive procedure called prostate artery embolization or PAE to treat enlarged prostate. This treatment cuts off some but not all of the blood supply to the prostate, causing it to shrink in size. Our partners Dr. Wood and Dr. Sill, interventional radiologists at Advantage IR, can complete this procedure by going through the arteries (instead of the penis). 80% of our PAE patients report that they can reduce or go off of their prostate medication and have more robust erections afterward! In this segment, Dr. Mistry and Donna Lee discuss an issue that affects both men and women, but that is particularly common in ladies--urinary incontinence. There are two kinds of "leaking" that can occur: stress incontinence and urgency incontinence. Stress incontinence is the bane of the new mommy, the aging woman, and those who have recently had abdominal-area surgeries in particular. This type of leakage is most often seen when women sneeze, cough, squat, run, or put some other type of "stress" on their bladders and urethras, which for a variety of reasons have likely started sliding down. Stress incontinence can be treated with pelvic floor physical therapy and sometimes surgery. Surgical interventions include Votiva, a treatment using radio frequency directed at the urethra to help strengthen that tissue and help with vaginal moisture to boot, and the mid-urethral sling, which is made of safe and robust mesh or even natural tissues. The other, more "urgent" type of incontinence requiring treatment is, of course, urgency incontinence. This kind of leakage is caused by an uncontrolled contraction of the bladder that makes getting to the bathroom in time very difficult if not impossible. Urgency incontinence can be treated with pelvic floor physical therapy, medication, or Botox injections or Axonics therapy--both of which calm the bladder down. More frequent, heavier leakage like that often seen with urgency incontinence needs to be treated by a urologist before rashes and other issues develop that seriously irritate this delicate area. In this segment, Dr. Mistry and Donna Lee answer are joined by guest Dr. Andrew Cassidy of Lone Star Podiatry to talk more about our feet! Today, Dr. Cassidy tries to help Dr. Mistry solve the problem of his four boys' stinky feet! According to Dr. Cassidy, foot fungus is both common and surprisingly difficult to treat. Fungus obviously thrives in dark, damp environments like shoes, which makes it likely to reoccur. Toenail fungus is also pernicious. Because the fungus itself grows up through the nail, topical solutions applied to the nails often fail to penetrate deeply enough to truly eradicate the problem. Oral anti-fungal medications like Lamisil work better, but come with their own concerns, like liver toxicity levels that have to be monitored. At Lone Star Podiatry, Dr. Cassidy does "pulse dosing" of oral anti-fungals which lower the risk of toxic accumulation while boosting effectiveness. There are also great over-the-counter treatment options like Dr. Cassidy's favorite astringent, anti-fungal powder, which can be sprinkled in the shoes. Using moisture-wicking socks made of synthetic materials like Lycra and spandex is also helpful in cutting bacterial and fungal growth. Finally, machines like SteriShoe are ultraviolet lights that kill bacteria when placed in the shoes to keep you from re-infecting yourself with foot fungus once you've tackled the source. While foot and toenail fungus seems like a non-threatening (if exceedingly icky) issue, these kinds of problems can be very dangerous for the elderly, people with compromised immune systems, and diabetics. Infections that cause wounds can be difficult to heal and can then spread to bones, which DON'T heal, but must then be cut out, causing even more risks.
