This program will discuss Transitioning from Pediatric to Adult Care. The transition from pediatric to adult health care is a complex process, particularly for adolescents and young adults with chronic health conditions and intellectual and developmental disabilities. In this presentation, learners will expand their understanding of the transition process, with particular attention on the roles of doctors caring for young adults in the transition process and get an introduction to skills and resources that can support young adults as they enter adult health care.
