"I feel it right in my joint!" It's a common complaint from many patients. Deep, specific pain. In most cases, an x-ray or MRI isn't needed an only leads to poor outcomes secondary to increasing fear avoidance. We try our best using patient education, and active strategies to keep people moving. In some cases, a scan is useful.

There are times when you know you have to address a patient's fear avoidance and maladaptive beliefs in order to make a difference in their pain and function. Sometimes you can try your best with many different ways of modulating pain, changing perception and reconceptualizing the patient's core beliefs about their issues. You can try your best, and talk patients out of scans... but on occasion, they're correct and they do need the MRI.

This is an unexpected part 2 from our last episode - Sometimes You Just Have to Rant! While Erson was impressed at the patient's acceptance of how sore she was after treatment, he didn't expect a simple screen and solution to it. How often do you screen after manual treatment if function is improved but soreness is increased?
Sometimes you can try your best, give your best positive message and you will be totally derailed by other clinicians giving a TERRIBLE negative message. Luckily, the patient's head was in the right place, despite her former PT and current ortho surgeon telling her to keep limiting her function, despite her continued improvements. Honestly, what are some providers thinking?
