What If I Take Naltrexone, But Don’t Feel Like Drinking? The Sinclair Method | Dr Joseph Volpicelli

The medication reduces cravings and helps extinguish conditioned responses to alcohol-related cues, even when no drinking occurs. Dr. Vachelli explains the mechanism behind this effect, highlighting how Naltrexone blocks the reinforcement of drinking behavior and also diminishes cravings triggered by environmental cues, such as being in a bar or watching Netflix at home. Over time, these cues lose their power, allowing individuals to be in high-risk drinking situations without temptation. Importantly, the medication also helps expand sources of pleasure beyond alcohol, such as enjoying a cup of hot cocoa or the company of a partner.

Furthermore, Dr. Vachelli emphasizes the importance of psychosocial support—including coaches, peers, or recovery groups—as a critical complement to medication. Such support systems provide accountability, encouragement, and hope, which are key to long-term recovery. Overall, the Sinclair Method, combined with Naltrexone and social support, offers people choice and freedom from addiction rather than strict abstinence or dogma.
