I explain why accountability systems fail even when everyone understands and agrees with the process. The problem is usually not a lack of intention. The problem is that no one is enforcing the accountability system when people fail to follow through. I've learned that accountability only works when follow-through is required, not optional, because accountability and optionality cannot exist in the same place at the same time.
