    In this podcast, Dr. Kelly Tappenden interviews Dr. Liam McKeever, ASPEN Clinical Guideline Director and Editor-in-Chief, discussing the new publication “Guidelines for the provision of nutrition support therapy in the adult critically ill patient: The American Society for Parenteral and Enteral Nutrition”. Dr. McKeever describes the background and methodology of the guideline and elaborates on the clinical recommendations for the 5 questions related to providing nutrition support in the adult critical care patients addressed in this guideline. Plans for a sister paper that will address questions not addressed in this updated guideline is also discussed.

  • Member Spotlight: Paul Wischmeyer, MD, EDIC, FCCM, FASPEN
    This video honors Paul Wischmeyer, MD, EDIC, FCCM, FASPEN, Professor of Anesthesiology and Surgery, in the division of Critical Care Medicine at Duke University in Durham, NC. Dr. Wischmeyer’s nutrition journey began in high school when he was diagnosed with inflammatory bowel disease and was on parenteral nutrition for a year. Many of the medicine he was given had severe side effects. It motivated him to get into medicine and do research on alternative treatments. Dr. Wischmeyer’s first grant was from the ASPEN Rhoads Research Foundation; it set him up for his NIH K Award. Dr. Wischmeyer provides tips for finding mentors in clinical nutrition, talks about an online nutrition fellowship at Duke, and shares what he thinks are the pressing questions in clinical nutrition.

  • Member Spotlight: Jose Pimiento, MD, FACS
    This video honors Jose Pimiento, MD, FACS, Associate Professor of Surgery, in the division of Gastrointestinal Oncology, at the Moffitt Cancer Center in Tampa, FL. Dr. Pimiento had the honor of training with Dr. Stanley Dudrick, the father of intravenous feeding. He shares some of Dr. Dudrick’s philosophy that as a surgeon, you need to be capable of handling everything, including the nutrition of your patients. Dr. Dudrick was upset with the approach of waiting for the next day to feed patients— tomorrow the patient will not have tests and will eat better, and then tomorrow comes and you’re doing something else on the patient, and so on. He was surprised that every day, we would replace electrolytes, but never think about replacing calories and protein. Dr Pimiento also talks about his innovative research work in nutraceuticals and cancer chemoprevention. He ends by sharing some advice for those starting out—finding interdisciplinary, local, and global networks of people with similar interests.

  • Update on Medicare Reimbursement Coverage for Home Parenteral and Enteral Nutrition
    In November 2020, the Durable Medical Equipment Medicare Advisory contractors retired the Local Coverage Determinants (LCDs) for home parenteral and enteral nutrition (PEN). On September 5, 2021 new LCDs took effect after close to 4 decades of no change. This podcast discusses the new LCDs, what it means for home PEN patients and home providers. It also addresses the challenges that still exist in home PEN Medicare coverage which will require advocacy from ASPEN, its partners such as NHIA, home care clinicians and patients.

  • ASPEN Lipid Injectable Emulsion Safety Recommendations, Part 2: Neonate and Pediatric Considerations
    Lipid injectable emulsions (ILEs) are complex pharmaceutical formulations intended as a source of energy and fatty acids for parenteral nutrition (PN). Part 1 of this series addressed adult use. Part 2 addresses ILE safety in neonatal and pediatric patients, who often require higher doses compared with adults to support growth, development, and daily metabolic needs. ILE is often administered as a separate infusion owing to compatibility and stability issues and limitations of access. It is associated with PN errors in the administration, prescribing, and transcribing processes and has been associated with adverse effects, including effects from doses exceeding manufacturer recommendations. This paper describes safe use of ILE products for US neonatal and pediatric patients, including indications, prescribing, order review, preparation, administration, and monitoring.

  • Member Spotlight: David S. Seres, MD, ScM, PNS, FASPEN
    This Spotlight honors David S. Seres, MD, ScM, PNS, FASPEN, director of the Medical Nutrition Program and associate professor of medicine in the Institute of Human Nutrition at Columbia University Medical Center in New York City. He has over 30 years of experience in nutrition support consulting, clinical research, and teaching. He’s currently the section editor for nutrition at UpToDate. Dr. Seres shares his thoughts on how the field of nutrition has changed, and not changed (lack of randomized control trial data), and what’s to come (personalized nutrition). He leads one of the few nutrition fellowship programs and points out that in the ICU, patients on average receive only 45% of the calories prescribed to them. He believes that ASPEN is an excellent way for young physicians to develop relationships, find mentors, develop their own credibility and have the opportunity to critically discuss the nutrition research to help further advance the field.

  • Malnutrition Diagnoses and Associated Outcomes in Hospitalized Patients - NCP - October 2021
    This podcasts discussed a sentinel paper, Malnutrition Diagnoses and Associated Outcomes in Hospitalized Patients: United States, 2018, looking at the most current HCUP (Healthcare Cost Utilization Project) data on malnutrition in hospitalized patients. The data confirms that malnutrition is prevalent in hospitalized patients and is associated with higher readmission rates, inpatient mortality and costs. The author discusses how this data compares with prior years and what it tells us about current clinical practice in screening, assessing and coding for malnutrition.

    Dr. Isabel Correia discusses her paper, "A Nutrition Therapy Cost-Effectiveness Model Indicating How Nutrition May Contribute to the Efficiency and Financial Sustainability of the Health Systems."
    Malnutrition is highly prevalent in hospitalized patients but seldom recognized and treated. Malnutrition poses several adverse events, such as increased infection rates, length of hospital stay, and mortality, as well as costs. Early nutrition interventions have been shown to decrease the associated malnutrition burdens, leading to relevant savings. This podcast discusses how this study aims to evaluate the cost-effectiveness of nutrition therapy, including oral supplements to at-risk or malnourished adult inpatients admitted to the Brazilian Public System (SUS) hospitals.
