This podcast discusses a study which evaluated parenteral nutrition (PN) use in a surgical subspecialty practice during a 6-month period evaluating the concordance with recommendations and to identify opportunities for i quality improvement. The authors discuss why PN use is frequently not concordant with established ASPEN recommendations. This leaves opportunity for quality improvement efforts focusing on appropriate initiation of PN based on malnutrition status & anticipated length of PN need. The discussion includes the team’s ongoing efforts to implement practice change.

Also highlighted is the complexity of the home PN patient which necessitates the management of these patients by trained nutrition support clinicians who are focused on individualized care.

This Spotlight honors ASPEN’s 42 president, Nilesh Mehta, MD, FASPEN, anesthesia professor and critical care nutrition & metabolism chair at Harvard Medical School in Boston, MA. During his early years of practice, Dr. Mehta realized there was very little evidence-based guidelines to drive nutrient delivery to sick children at the bedside. Depending on the team, the child was fed differently and the fundamental question of how much food/calories to deliver was unanswered. This began his journey into research.

Dr. Mehta stresses that nutrition in the ICU is a team effort and that bedside nurses are his partners, the drivers of change. He shares how ASPEN has all the things he needed—interdisciplinary teams, mentors, and collaboration —and how a call to ASPEN got him started working on guidelines.

In this podcast, Editor-in-Chief Jeanette Hasse, PhD, RD, FADA, CnSC, interviews Ryan Spellecy, PhD, and Kristin Busse, PharmD, BCPS.

This Spotlight honors Robin Mendelsohn, MD, clinical director of Gastroenterology, Hepatology and Nutrition Services, and associate professor of clinical medicine at Memorial Sloan Kettering Cancer Center (MSK) in New York City. Dr. Mendelsohn’s interest in nutrition was solidify after she had 6 months of nutrition focus in her GI fellowship at MSK. Today, she runs the nutrition services at MSK and offers a dedicated nutrition fellowship. She notes that when she first started, obesity management was not a focus when treating cancer patients even though obesity is a risk factor for so many cancers. But now patients are living longer with cancer and obesity management is incorporated into the treatment.

Dr. Mendelsohn is also co-director at the Center of Young Onset Colorectal Cancer where they provide coordinated care for patients under 50 years of age. She says many want to learn how nutrition can get them through treatment and survivorship. She’s doing research to see why colorectal cancer is increasing in younger people, it’s if caused by diet and/or environment.

This Spotlight honors Renee Stapleton, MD, Professor of Medicine, Division of Pulmonary and Critical Care Medicine, Burlington, VT. When Dr. Stapleton was a fellow, she became interested in research and saw nutrition in the ICU as an open box, with many unanswered questions. She has since led multiple nutrition RCTs, including a trial examining the effect of combined exercise and protein delivery in critically ill patients. Her advice to trainees with a research interest is to find good mentors, learn from the research coordinators, focus on developing strong writing skills and invest time to take workshops on grantsmanship and publishing.

Spotlight honors Carol E. Semrad, MD, Professor of Medicine, Director of Adult Clinical Nutrition and Small Bowel Disease, Section of Gastroenterology, Hepatology and Nutrition at University of Chicago Medicine. Dr. Semrad highlights the limited nutrition training in GI programs and the importance for physicians with interest in nutrition to find mentors who are experts in the field. She emphasizes the need for more clinical nutrition experts and resources for nutrition programs. As she discusses her own nutrition support team and how they have expanded to manage in-patients and out-patients, she shares how gratifying it is to see nutrition support restore nearly starving patients to health and allow them to lead more functioning lives.

Spotlight honors Sara Bonnes, MD, MS, Associate Professor of Medicine, Division of General Internal Medicine at the Mayo Clinic. Dr. Bonnes found her passion in nutrition early and credits her knowledge in nutrition for opening up opportunities for her. As a General Internist, nutrition is pervasive in her practice, she sees so many diseases where nutrition is a key component in the management or the onset of the disease. She works in Mayo’s home PN program where she and her multidisciplinary team provide long-term care to patients on PN. Dr. Bonnes’ one advice to PN clinicians is to reach out to team members and peers for help. She says ASPEN is part of her team.
