    The JPEN article discussed in this podcast highlights the POET program developed and implemented at Duke University to address the issue of preoperative malnutrition and its negative consequences. The paper’s author discusses the preliminary experience to efficiently screen patients for preoperative nutrition risk and proactively manage malnutrition before and after surgery. The POET team believes that programs such as this will improve nutrition screening, nutrition care and outcomes, along with significant cost savings due to the negative and costly effects of malnutrition.

  • Vitamin E Levels in Children Treated with an Intravenous Fish Oil Lipid Emulsion.
    The study discussed in this podcast was an Abstract of Distinction from ASPEN20. Due to a concern of potential vitamin E toxicity in patients receiving a fish oil (FOLE) or mixed lipid emulsion containing fish oil, which has added vitamin E to prevent oxidation of PUFA, the study reviewed vitamin E levels in children being treated for IFALD with FOLE. In this podcast, the study author discusses the details of the study and the basis of the conclusion that there is low concern for development of hypervitaminosis E toxicity with FOLE therapy in this patient population.

    This podcast highlights an abstract from ASPEN20 in which a study looking at trends of liver profiles, micronutrients, nutrition, and growth for pediatric intestinal failure patients undergoing intestinal rehabilitation while using a mixed oil intravenous lipid emulsion with the goal of providing up to 30% of parenteral nutrition calories from lipids. The study was successful in delivering up to 30% of calories from lipids, allowing a decrease in glucose infusion rate and an increase in concentration of kcals per hour reducing overall hours per day of TPN. No patient progressed to cholestasis and growth and development was supported.

  • A Retrospective Evaluation of a 4-Oil Lipid Emulsion on Liver Function for Home PN Patients
    This podcast highlights an ASPEN 20 Poster of Distinction of a study which assessed the safety and tolerance of a 4-oil vs soybean oil (SO) lipid emulsion in adult HPN patients. The study demonstrated that 4-oil lipid emulsions were a safe and effective alternative to SO emulsions for HPN patients. Switching from SO to a 4-oil lipid emulsion as a major component of HPN was associated with a significant improvement in serum total bilirubin and alkaline phosphatase levels.

As a result of this review the indication for use of this new ILE was decreased from an estimated or actual PN duration of greater than 30 days to a duration of 14 days, which the authors stated better meets their patient needs.
