DRIVE TIME DEBRIEF: A Physician Wellness Podcast with The Whole Physician episodes

  • #9: Thought Errors 5/5

In this episode, we continue with the 4th of the 5 part series on the 15 most common thought errors. In episodes #3-4 we went over how to try to identify the thought that is causing your negative (or positive) feelings. After you’ve processed the feeling, it’s important to look at if the causative thought is even true, useful in any way, or if there are any other alternatives to consider.  This series starts to examine the thoughts- to consider if it falls within any of the 15 most common thought errors.

    Distortions 13-15/15:

    • Fallacy of change - when we think others need to change in order for us to be happy. In reality, we are responsible for our own happiness. We can’t control other adults, so needing them to be other than who they are is an exercise in frustration.
    • Fallacy of fairness - getting stuck because something isn’t fair or equal. The truth is living on earth has rarely or ever been fair
    • Heaven’s reward fallacy - expecting rewards based on merit. Since in reality things are rarely fair, we need to live in accordance with our own integrity rather than expecting acknowledgement, promotion, etc for good deeds or hard work.
    • QUOTABLES:

      • “The best gift I can give myself is loving myself unconditionally in the same way that we were needing that validation from a supposed or partner…”
      • “One of the most important things that we could do for ourselves…was to start loving ourselves as we are.”
      • “When you fight reality, you lose every time.”
      • “We decide who we are, and that’s how we show up, whether we get reward or appreciation or not.”
      • “If you’re motivated to do the things that you do expecting some sort of return on it, then that’s an exchange.”
      • “As we do that and create our own reward, we’re less reliant on- we are less likely to transfer our power to somebody else, expecting them to make us feel good.”
      • "It is all about becoming the person that you know you can be."
      • "You are enough, and your worth is not in what you do or what you say or what you accomplish."

      • #8: Thought Errors 4/5

In episodes #3-4 we went over how to try to identify the thought that is causing your negative (or positive) feelings. Distortions 10-12/15:

        • "should statements" - statements like "I should be exercising more" create expectations that are not likely to be met. The pressure created from the “should statements” makes it difficult to meet those expectations, when the failure occurs there is guilt and frustration, which makes you less likely to make another attempt.
        • emotional reasoning - any feeling must be true in their mind. The emotion is accepted as fact because all logical reasoning is blocked out and it incorrectly assumes the negative feeling is the only truth.
        • control fallacies - (2 subtypes)
          • external control fallacy is the belief that a person's life is completely controlled by external factors and fate has already been decided. This distortion creates the feeling that we have no control over our situation.
          • internal control fallacy is the belief that a person has complete control, not only of themselves, but also their surroundings. So, you can assume that you are responsible for not only the pain of yourself and others, but also their happiness.

            Distortions 7-9/15:

            • Blaming - holding others responsible for our pain, playing the victim role
            • Labeling - assign judgment to ourselves or others based on one negative incident
            • Always being right - believe our opinions are facts, NEED to be right, efforts to prove ourselves right persist well beyond when most would “agree to disagree.”

                • Jumping to conclusions - (2 groups)
                  • Mind reading-you think you know what others are thinking, assumptions of their intentions occur with no evidence.
                  • Fortune telling-which is making or make conclusions and predictions with no evidence and can have negative outcomes.

In this episode we start a 5 part series of the 15 most common thought errors.  In episode #3 we went over how to try to identify the thought that is causing your negative (or positive) feelings. After you’ve processed it, it’s important to look at if that thought is even true, useful in any way, or if there are any other alternatives to consider.  This series starts to examine the thought to consider if it falls within any of the 15 most common thought errors.

                          First up, thought errors 1-3:

                          • Polarized thinking - this is an all or nothing thinking, ignoring shades of gray
                          • Mental filtering (which is broken up into 2 subtypes):
                            • Negative mental filtering - one negative thing ruins the rest of the day, project, etc
                            • Disqualifying the positive - dismissing an accomplishment in one way or another
                            • Overgeneralization - making assumptions about the future based on a single interaction/event

                            • #4: How to Coach Yourself

In this episode, we talk more about bringing awareness to what’s going on in your brain and how it’s ultimately creating the results in your life. Additionally, we start to discuss starting to think about changing the sticky thoughts giving us problems.

                              The Cognitive Thought Model

A brief review of the cognitive thought model. Anything that happens outside of you and your control is a situation or CIRCUMSTANCE (C). It is by definition neutral and a fact that everyone agrees upon. It isn’t until you have a THOUGHT (T) about the circumstance that you have a FEELING (F). It’s actually the feeling that drives your ACTION (or inaction or brain chatter). The sum total of your actions/inactions is what gives you the RESULTS (R) of your life.

                              • Start with a daily thought dump - keep it raw and unfiltered (you can always burn the paper later). (See our handout below for a template.)
                              • Separate out the facts, the rest are just thoughts.
                              • For any problematic thoughts, take a close look to evaluate:
                                • why you’re choosing that thought
                                • how it makes you feel
                                • if it’s serving you in any way to keep that thought
                                • are there any other valid perspectives
                                • Once you’ve really evaluated the thought - it’s a good time to have compassion for where the brain learned this and why it offered it up. You can become aware of it and allow it to pass in the future. And if you ultimately decide this thought isn’t helpful for your life, you can move toward an intentional model.

Victor Frankl famously said, “Between stimulus and response there is a space. In that space is our power to choose our response. In our response lies our growth and our freedom.” In this episode, we explore that space.

                                  Key Quotes:

                                  • “Metacognition happens anytime we have awareness about our learning, thinking and other cognitive processes. But from a deeper level, metacognition involves meta-awareness, truly detaching from the mind to get the bird’s eye view from above.”
                                  • “There is a step between all of the things outside of us and our feeling, and that’s our thoughts.”
                                  • “We all think that we want to be happy all the time. That’s not true.”
                                  • “If that feeling is working for you- feeling frustrated or disappointment or angry…no one’s taking that away from you. It’s your life; you get to do whatever you want.”
                                  • “They can still be them, and it doesn’t have to affect us at all.”
                                  • “No longer does anybody have access to what’s in between my ears.”
                                  • “Jon Kabat Zinn is the mindfulness guy. He says the point is to be aware, on purpose, without judgement. It’s just information.”
                                  • “The thoughts are not to be resisted…There are no forbidden thoughts. They’re not good or bad. If they’re getting you where you want to go, then keep it. If they’re not getting you where you need to go, you can allow it to pass….And then intentionally choose a different one that’s a little more helpful for you.”
                                  • “There’s no more judging of yourself.”

In this episode, Drs. Morrison, Cazier and Dinsmore introduce themselves and discuss their own personal wellbeing struggles while working in medicine (spoiler: 2 of the 3 even quit at one point to save their sanity). After the lowest points in their careers, they each began a journey back to wellness which has enabled all three to continue working clinically. Currently, after additional training in life, health and wellness coaching, they’re ready to share all they’ve learned in the hopes that some of the suffering they experienced can be avoided by others.

Get ready to meet Dr. Kendra Morrison, Dr. Laura Cazier, and Dr. Amanda Dinsmore.

                                      Some highlights from Dr. Dinsmore:

                                      • “I had a harder and harder time forgiving myself for being human.”
                                      • “ I became a professional feeling-stuffer, and I was using food to help with feelings. I was using wine. I was using television, like binge-watching all of the Real Housewives of every city.”
                                      • “I was ruminating; I wasn’t sleeping. I was thinking about everything that went wrong all of the time.”
                                      • Some highlights from Dr. Morrison:

                                        • “At this point in my career- is this it? It can’t be it. And I can’t be this angry all the time. I can’t be this disappointed.”
                                        • “I think all of us as physician find ourselves in that position where we can not disclose. We feel that there could be some report to a medical board, or just fear of having a stigma of having some kind of mental health diagnosis.”
                                        • “Everything that we have learned over the last couple of years…has overflowed into all of my relationships and has been rewarding.”
                                        • Some highlights from Dr. Cazier:

                                          • “And I know, I know most of us have had that feeling where everyone’s just way too sick and everyone is spread way too thin. And it’s just scary. And I got to the point, I was like, what am I doing? This is horrible. I am shortening my life, and I’m miserable. I don’t like the person I am.”
                                          • “I remember the discussion I had with my medical director before I quit. She was like, you may never be able to come back. And I was like, I don’t care. I don't care if I never see this place again. And at that time I meant it.”
                                          • “We’re all resilient already. Otherwise we wouldn’t have made it through residency training, but to really come into any circumstance and know how to manage our minds around it is a tremendous skill that I’m continuing to develop.”
                                          • “There’s nothing more powerful than to create a beautiful life that is the life that you want.”

Physicians are feeling the burn of working in medicine. In this episode, Drs. Morrison, Cazier and Dinsmore review the 2022 Medscape article, “Physician Burnout & Depressions Report 2022: Stress, Anxiety, and Anger." This survey of over 13,000 US physicians revealed:

                                            • which specialities are the most affected by burnout
                                            • male vs. female burnout rates
                                            • what physicians reported contributes most to burnout
                                            • severity of impact burnout has on their lives
                                            • quotes from doctors in their own words
                                            • most commonly used coping strategies
                                            • evidence for reluctance to seek help
                                            • willingness to seek out professional help going forward
                                            • Quotables:

“Emergency Medicine, Critical Care, OB/GYN, Infectious Disease, Family Medicine, Physical Medicine & Rehabilitation, and Diabetes & Endocrinology were all 50% or greater burned out. So being alive and being one of those doctors working, you are more likely to be burned out than not.”

“It’s no wonder that people don’t seek help when they need it.”

“I think the medical culture definitely reinforces being impenetrable. If you have a problem, don’t talk about it.”

“There are things that we can do with simple mind shifts, mindset shifts that can make such a tremendous difference. And that’s why I’m excited.”
