In this segment, I start with a historical monologue on the history of depression and its' treatments throughout human history. Then, Dr. Gillman explains why it's so important to adequately treat depression with appropriate medications, despite sometimes having to manage a more severe side effect profile. He argues that the risk/benefit ratio, especially when biologic (vs. neurotic) depression is appropriately identified and diagnosed, oftentimes reveals greater benefit with MAOIs and TCAs than with SSRIs.

Dr. Gillman gives us some fascinating historical background discussing the emergence of the medical publishing industry and its' relation to Robert Maxwell, the father of Ghislaine Maxwell (confidant of Jeffrey Epstein). He talks about how decisons are driven by financial incentives and discusses how rating scales and data manipulation runs rampant in all of medicine, but especially in psychiatry, and how research can be very unreliable in the current publishing climate.

In this segment, Dr. Gillman and I return to the drawing board and discuss on a very fundamental level, what depression is. We talk about how the concept of and diagnosis of depression has been progressively watered down over the last 50 years, leading to more people being diagnosed and treated with SSRI antidepressants, which have very questionable efficacy and under-reported and under-recognized side effects.

This is the introductory episode to our opening series on Antidepressants - Going Backwards with Dr. Ken Gillman. We'll talk about how 1st line antidepressant medications have become less effective with time, how the diagnostic criteria for depression have been watered down, and reveal how Evidence-Based Medicine can be heavily manipulated.
