  • EP 732 - Jeff Lyth and Brent Sutton

In this special episode recorded live at the Energy Safety Canada Conference, Brent Sutton and Jeff Lyth deliver a compelling keynote on how the 4Ds—Dumb, Dangerous, Difficult, and Different—highlight the real-world challenges faced in day-to-day operations.

They explore how organizations are applying Human and Organizational Performance (HOP) principles to bridge the persistent gap between work as imagined and work as done. The conversation evolves into the role of AI-powered tools in scaling operational learning, anticipating emerging risks, and designing better work for people.

Brent and Jeff also address the growing AI dilemma—how we ensure trust, accountability, and fairness in AI-driven systems while navigating the tension between human error and technological advancement.

At the conclusion of the session, Jay Allen had the chance to sit down with both speakers for a brief interview to reflect on the message and what it means for the future of work.

  • EP 728 - Steve Scott

Today on the show, we listen to a presentation by Steve Scott that took place at Energy Safety Canada. Here is the content of the presentation - Many organizations have operated under the principle that "zero incidents" is the only acceptable outcome, believing every incident is preventable. Despite these efforts, serious injuries and fatalities still occur.

Join Steve Scott in a session that challenges long-held safety beliefs and explores a new approach to protecting workers. This session will introduce the first principle of Human and Organizational Performance: Error is Normal.

Instead of focusing solely on preventing injuries, what if we shifted our attention to protecting workers, even when things go wrong?

Learn more about Steve’s transition from a "zero tolerance" approach to a focus on "failing safely" – and what that shift could mean for the future of workplace safety.

Don't forget to listen to the convo between me and Steve Scott at the end of his presentation.
