We tick off our last universal principle to overcome your proximal hamstring tendinopathy. Brodie introduces the Pain, Rest, Weakness Downward Spiral which is a common trap for tendon pain.

It is human nature to retreat from activity if it causes a flare-up, but in reality, if you are constantly removing aggravating factors without replacing it with a proactive exercise, the tendon becomes weaker and less able to handle rehab exercises.

Brodie dives into detail and how you can use this concept to your advantage.

This universal principle focuses on the difference between external tissue loading & internal tissue capacity. In particular, your adaptation zone & max capacity line is not static. In fact, certain environmental influences and mood states can fluctuate this line and influences how much load your proximal hamstring tendon can take!

Brodie talks through the chapter of his e-book and then chimes in at the end to mention the importance of your fight or flight mode and your rest & recover mode.

We dive into another chapter of the E-book and introduce you to another universal principle to overcome your proximal hamstring tendinopathy.

The chapter of the e-book is titled: 'Sleep on it: Interpreting your pain over 24 hours'. Your tendinopathy recovery will be drastically disabled if you cannot accurately interpret symptoms. What makes matters tricky, is sometimes pain can be a good thing. Brodie uncovers this dilemma on today's episode.

On today's episode, Brodie dives into the stages of PHT rehab. We first touch on the responsibility of pain during rehab exercises, and how we can interpret productive pain from unhelpful pain.

Next, we discuss the 3 stages of rehab. Stage #1 includes how to settle down pain from a reactive flare-up, including the appropriate use of isometrics, daily load modifications and medications.

Stage #2 covers different levels of strengthening exercises. Brodie explains how the tendon can start 'out of compression' and slowly build on compressive loads throughout the rehab process. Certain exercises include hamstring curls, Swissball bridging, swissball rollouts, deadlifts, lunges & squats.

Stage 3 incorporate speed, power & addressing the whole kinetic chain with exercises like KB swings, rocket jumps, plyo lunges, wall balls & single leg deadlifts.

Lastly, Brodie touches on the psychosocial factors of your rehabilitation and how treatment and be influenced by beliefs, thoughts and emotions.

    We break rehab into stages:

    • Stage 1: Identify daily compression and reduce if required.
    • Stage 2: Start tendon loading & monitor response. Including isometrics, eccentrics, concentric/eccentric, power, speed.
    • Stage 3: Return to sport & monitor symptoms
    • Stage 4: Start addressing entire kinetic chain while returning to activity.

Brodie then chimes in with his 5 key injury prevention tips & comments on the Proximal Hamstring Tendinopathy insights.

Understanding Proximal Hamstring Tendinopathy (PHT): Causes, Symptoms & The Right Recovery Approach

If you’ve recently discovered this podcast because you’re dealing with deep buttock pain, pain with sitting, or a stubborn “high hamstring” issue that just won’t go away… you are not alone.

In this updated foundational episode, I break down everything you need to know about proximal hamstring tendinopathy (PHT): what it is, why it develops, why it’s so commonly misunderstood, and what successful rehab actually looks like. I also share parts of my own journey overcoming PHT and the common patterns I now see after helping hundreds of runners navigate this condition.

    We dive into:

    •  The true causes of PHT and why overload + recovery balance matters
    •  Why speed work, hills, sitting and compression can flare symptoms
    •  The “warm-up effect” and why tendons can feel better during a run… then worse the next day
    •  Common misdiagnoses including sciatica, piriformis syndrome and back-related pain
    •  Why stretching and complete rest often fail
    •  The difference between reactive vs degenerative tendons
    •  How modern rehab focuses on gradually rebuilding tendon capacity rather than avoiding movement altogether

Whether you’re newly diagnosed or have been battling PHT for years, this episode is designed to give you clarity, direction and hope. There is a path forward — and understanding the condition is the first step.

Brodie starts by breaking down tendon pathology definitions such is proximal hamstring tendinopathy, tendinosis & tendinitis.

We then explore the pathology continuum, originally proposed by Jill Cook & colleagues while explaining the progression of tendon pain and the end result if not managed correctly.

Next, Brodie covers the common locations of tendon pain & why we get a tendinopathy which will include spike in tensile and compressive loads.

Finally we tick off all the main risk factors associated with developing a tendinopathy.
