Sophie Cunningham lit the internet on fire when she said we need to protect the safety of girls and the integrity of women's sports, and the conversation quickly turned into another political battleground even though the underlying question is really about biology, anatomy, and physiology. Inside today's episode, I break down what the research says about testosterone suppression, puberty, and the physiological differences between men and women that don't disappear just because testosterone levels come down.
Topics discussed:
- A Brief History of Trans Athletes in Sport
- Pre-Puberty Differences Between Boys and Girls
- What Male Puberty Does That Can't Be Reversed
- What Testosterone Suppression Does and Doesn't Change
- Bone Density, Muscle Mass, and Neural Capacity

Roughly 1 in 4 American adults are taking 5 or more prescription medications at the same time, and that number climbs steeply for people over 40, for postpartum women, and for anyone managing more than one chronic condition. The most talked about murder trial of the year has put a public face on what polypharmacy looks like at its most extreme. I break down the polypharmacy conversation, walking through why national health systems in the UK, Scotland, Ireland, and the US have all written formal frameworks for managing the medication burden, why that alone tells you this is the standard rather than the exception.
Topics discussed:
- The 1 in 4 American Polypharmacy Statistic
- National Frameworks for Managing Medication Burden
- The Prescription Cascade in Younger Women
- The Endocrine and Psychiatric Overlap
- Postpartum Thyroid Conditions and Depression
- The Thyroid-to-Prolactin Cascade
- Nutrient Deficiencies That Look Like Depression
- The Lab Panel to Run Before Prescribing

Roughly a quarter of American adults are taking five or more prescription medications at the same time, and that number climbs steeply past age 40. It shows up in national survey data, in Medicare data, and in European population studies, so it's a feature of the Western medical world, not a quirk of one system. Health systems have written entire national guidance documents whose only job is managing the resulting polypharmacy, which is the clearest sign that this is the standard rather than the exception. The most talked about murder trial of the year has put a public face on what polypharmacy looks like at its most extreme, but the everyday version of this story is playing out in millions of primary care offices, OB follow-ups, and more. Inside today's episode, I break down the polypharmacy conversation, walking through why national health system have all written formal frameworks for managing the medication burden, why that alone tells you this is the standard rather than the exception, and how the cascade of prescriptions tends to unfold.

nside today's episode, I break down the emerging research on how high blood sugar affects the gut barrier and why hyperglycemia and leaky gut are more connected than most coaches and clinicians appreciate. I walk through the Danish observational study of 69,000 people with type 2 diabetes tracked over more than a decade, why cumulative A1C exposure predicted a 64% higher risk of hospitalization for infection while a single snapshot at diagnosis did not, and what that tells us about the difference between one lab value and a trajectory.

Your immune system is built to hunt invaders, but every so often it turns its weaponry on your own tissues. Over the last 50 years, autoimmune disease has gone from an uncommon condition to something almost everyone knows at least one person living with, and the two variables that move symptoms more than anything are almost never discussed in a 10-minute doctor's appointment. Inside today's episode, I take the umbrella view on autoimmunity, covering how the whole category works and what the underlying mistake looks like whether it lands on the thyroid in Hashimoto's, the joints in rheumatoid arthritis, the nerve coating in MS, or half a dozen organs at once in Lupus.
Topics discussed:
- The Rising Autoimmune Curve
- The Immune Balance Tipping Point
- Why 80% of Patients Are Women
- The Hormone Picture in Autoimmune Disease
- Oral Contraceptives and Autoimmune Risk
- Gut Microbiome and Immune History

Inside today's featured replay interview, I walk through how to assess metabolic health from both the subjective side of energy, sleep, and appetite regulation and the quantitative side of lab work, and why coaches are best thought of as adaptation managers rather than diet prescribers. I cover the five pillars I use for managing metabolism including managing appetite, maximizing adherence, maximizing absorption, minding micronutrient status, and mitigating metabolic adaptation, and why building these into a framework matters more than following any specific diet or influencer's plan.

80% of Americans are metabolically unhealthy, which means the obesity conversation isn't about weight for a lot of people... It's about internal health markers that indicate a declining state of metabolic function regardless of what someone weighs. Inside today's episode, I get into the harder version of the obesity conversation including whether it should be classified as a chronic medical condition, what happens when we label people as broken and then sell them the solution to their "brokenness", and where the personal responsibility conversation sits in a landscape flooded with GLP-1 medications and food environments engineered to defeat every mechanism the body has for satiety.
