Hey Jimmy & Adam,

I’m a sugar addict. My current streak of not eating sugar and other crappy carbage is 42 days, but the holidays are always the toughest when I struggle the most. Dessert is my biggest downfall and, unlike Dr. Nally who can get back on the wagon within a few days, it takes me a couple of months to recover. It was Thanksgiving, then Christmas, then New Year’s…and now Valentine’s Day, Easter, Fourth of July and more are coming up in the next few months. My willpower is very strong before the winter holidays, but I need a good strategy for staying strong through all these temptations. I’ve been looking for good dessert alternatives, but even maple syrup, honey, stevia, and the other sweeteners seem to set my cravings off. Is abstinence from all sweets altogether the only answer for someone like me?

1. What are the signs other than testing that my body is running primarily on ketone bodies?

Hi Jimmy and The Doc,

I enjoy your Keto Talk podcast and I have a question for you guys. I am mostly Paleo (eating the 80/20 lifestyle) but have always been curious about the ketogenic diet and have even tried it a couple times. But I haven't been able to stick with it for very long. I don't have any health issues (my insulin is normal, I’m at a healthy weight, I exercise regularly), but I am interested in maybe implementing some form of keto lifestyle for changing my body composition.

My question for you guys is when do you know you are in ketosis other than testing with a ketone strip or getting blood work done? What specific signals does the body give you to let you know ketones are your primary fuel source? I have noticed that when I am low-carb, high-fat, I can feel some change in my body in terms of energy and a spring in my step. But I also get a cold feeling running down the back of my throat. Is this normal when in ketosis?

2. Why isn’t my A1c going down on my ketogenic diet? Would taking Metformin help?

Hi Jimmy and Adam!

I love this podcast and I've learned a ton of information about ketosis along with reading Keto Clarity. I have Hashimoto's and was recently diagnosed with Celiac Disease. I am on Armour thyroid and my thyroid labs have been consistently doing well for the past few years. However, my A1c has been in the pre-diabetic range (5.7-6.0) for the last few years despite my best efforts to reduce it through diet. Two months ago I went keto hoping to bring it down while also treating my Hashimoto’s. Imagine my surprise when my latest labs show my A1c is still 5.8 despite keep carbs under 20 grams a day and carefully watching my protein intake.

Is 2 months not long enough for me to see real improvement in my A1c? My endocrinologist tested me for adult onset type 1 diabetes, but that came back negative. All my other blood work looks great except for the A1c and my liver panel. Could the liver abnormalities impact the A1c? I wonder if someone like me could stand to benefit from taking a low dose of Metformin to give an extra push with my ketogenic diet. I’m using the Ketonix to test for the presence of ketones in my breath and I’m almost always positive for ketosis. Do I just need to be more patient? I am fully committed to keto regardless, but just wondered about these other things.

3. Does surgical menopause increase carb cravings? Will keto prevent a recurrence of cancer?

Jimmy & Doc Nally,

I had surgical menopause in 2015 in response to persistent PCOS and early stage breast cancer and it gave me peace of mind that those cysts would not become cancerous. However, this surgery has caused other issues now. Most notably, I’m finding it harder to MENTALLY do keto because I feel like I am having more cravings and less willpower than ever before. I’ve done low-carb and ketogenic diets with great success previously, but menopause seems to have changed something in me. Did something actually change or is this all in my head? And do we have any evidence that a ketogenic diet will prevent a recurrence of cancer?

- How do I determine whether going ketogenic is right for me?

Hey Jimmy and The Doc,

I’m really grateful for discovering your podcast. I think we will look back on these past few years as the place where the truth about food and health began to come to the surface. I have been listening to KetoTalk and it has convinced me that going keto is the way to go for me. You guys have often said that you need to find what works for you and that a ketogenic approach isn’t necessarily for everyone.

My problem is if I want to get into full on keto mode, it sounds like that I need a doctor to help me get started who would be willing to help me to do something that many in that profession thinks runs counter to his belief about health. But Dr. Nally has clearly said that a ketogenic diet isn't for everyone, so here’s my question for you guys—how do I know whether or not it is right for me? Do I have to be metabolically broken and how do I know if I am? And if a ketogenic diet is not right for me, then what is?

It seems this message of low-carb, high-fat, ketogenic diets is still so fringe and it’s difficult to explain it to doctors, friends, and even my own family where my wife says if sugar was so bad for you, then why would they sell it in stores? This is an uphill battle that I face here in the real world and would love your help in navigating through it all.
