Episode 152 hosts Madilyn Wolens, a Registered Nurse based in Brisbane, Australia.
Better known as 'The Functional Nurse' and Founder of 'The Functional Skin Studio', Madilyn joins us in our 3rd Chapter of 'The Injector Diaries' - an in depth conversations, stories and experiences from injectors around the globe. In this mini-series, each injector brings their own unique take on things and we'll showcase every level of injector, from newbies to masters and from all walks of life. We'll explore how and why they chose to inject, why they favour using certain products, look under the hoods of their clinics and businesses and aim to learn from and inspire our injector listeners.
Madilyn suffered with poor skin as a teenager and was inspired to work within the aesthetic space to help others with similar issues. But her experience of conventional treatments sparked an interest in a more holistic approach to skin care and she now leads a team including a naturopath, holistic facialist in addition to offering injectables herself.
In this episode, we discuss:
- Her motivations for getting into the industry
- Her personal skin battles
- Originally training in the beauty sector before becoming a registered nurse
- Her early injectable career and how she trained
- The transition from a free lance injector to opening her own clinic
- The challenges of owning a clinic
- The impact our internal health has on our skin
- Working alongside complementary practitioners
- Recognising her skill set, working within her scope of practice and referring cases to others
Every week we also ask our Injector Diaries guest:
- What's your favourite toxin and why
- What's your favourite filler and why
- What's your go-to cannula make and size?
- Aspiration, yes or no?

Dr Jake & David share some insights into just how disruptive the effects of the lockdown have been for the podcast. They reflect back on their last episode together (Episode 141) where they made predictions about what the Australian injectable and aesthetics industry would look like post-lockdown. - Dr Jake shed's some insight into his role as an injectables trainer (and teases a new training opportunity for injectors in 2022!)
We then look forward to IA's future plans including:
- a huge Christmas special planned for Dec 24th where we talk to the inventor of the world's first hyaluronic acid filler, Restylane
- a look ahead at 'The Injector Diaries' & 'Treatments & regions uncovered' episodes and who we might talk to in the near future
- news about our YouTube channel (is it finally going to happen?!)

We also discuss the anatomy and facial ageing signs of the area to better understand what's happening before going on a deep dive to find out:
- how our guest does it
- what products and tools they prefer and why
- what approaches they use and why
- what we can learn from their experiences and the complications that they've encountered
As an extra twist to these episodes, our co-hosts Dr Jake and David will take on different roles! David will act as our main host and give his own 'patient' perspectives whilst Dr Jake will join the week's guest as a second expert injector to share his own insights, tips and tricks.
This week we'll be exploring the 'tear trough and peri-ocular region', the delicate zone below the eyes and an area commonly requested by patients - but notoriously difficult to treat well.
We cover:
-what exactly is the tear trough?
-when do patients typically start noticing changes in the under eye region?
- What are the factors that cause the tear trough region to age?
- How do genetic factors influence the tear trough including skin colour, race etc
- Are there preventative measures patients can take to slow down or prevent (to a degree) hollowness in this area?
- What does a "bad" tear trough treatment look like?
- Why is this area/treatment often misdiagnosed?
- Needle vs cannula techniques
In celebration of our 150th birthday and as a nod to our listeners and their feedback, we hope the introduction of this new series gives you a deeper dive into injecting and techniques. Our aim is to provide our listeners with as much value as possible whilst constantly evolving our content as our IA community grows.

Episode 147 hosts Dr Tim Brown (Plastic Surgeon based in Melbourne) and Dr Niro Sivathasan (Cosmetic Surgeon based in Sydney)
We invited these two surgeons from different backgrounds to discuss the current crisis in the Australian cosmetic surgery industry.
For some context to this podcast, it's currently legal for any doctor who holds a medical license to practice and perform cosmetic surgery - regardless of their medical expertise or experience. This unusual loophole can be exploited because there is no formal pathway for any doctor to train in cosmetic procedures.
For many years, there's been an uncomfortable and sometimes volatile relationship between plastic surgeons and other doctors who practice cosmetic surgery - who are collectively known under the umbrella term 'cosmetic surgeons'. This label covers a wide spectrum of doctors, all the way from first year graduates with no formal training through to highly respected and experienced cosmetic surgeons with 25+ years experience.
Just a fortnight ago, a national TV programme called 'Four Corners' re-ignited this debate with alleged unsafe and poor standards from a prominent and well known cosmetic surgeon. This is the crux of the issue that plastic surgeons and the public have increasingly voiced.
00:00 Introduction 01:04 Meet Our Guests: Dr Tim Brown & Dr Niro Sivathasan 05:42 Plastic vs Cosmetic Surgery Explained 11:31 Why Training Standards Are Being Debated 17:23 Lack of Regulation and Defined Competency 22:54 Media Coverage and Public Perception Risks 25:30 Examples of Clinic and Safety Concerns 29:56 Role of Regulators and Enforcement Challenges 33:11 Financial Incentives and Industry Tensions 36:56 How Patients Can Assess Surgeon Credentials 38:07 Social Media Marketing and Ethical Boundaries 39:55 Why Communication Failures Lead to Complaints 43:36 Importance of Consultation and Patient Selection 45:27 Saying No and Managing Expectations 47:24 Pathways Toward Better Standards and Collaboration 50:29 Final Reflections on Patient Safety and Industry Unity
