    Impelled by the pressures of climate change and population growth and shaped by the promise of technologies like 3-D printing, a revolution is brewing in the future of homebuilding around the globe — especially in cities.

    As Earth's inhabitants, we need to rethink almost everything about the way we live, especially in coastal cities, because our world may be reshaped by rising oceans in ways we can't yet fully anticipate, according to Hans-Peter Plag, a professor and director of the Mitigation and Adaptation Research Institute at Old Dominion University in Norfolk, Virginia.
     
    "We have built with the design principle that sea level is not going to change," he said. "This time has come to an end. On the other hand, living in the coastal zone is extremely important for us. So we need to find a way to live in the coastal zone, but we cannot assume that sea level is stable."

    Complicating the problem of a shrinking landmass is the prediction that the human population will grow to almost 10 billion people by 2050. And while many cities in the world will have to cope with extraordinary growth, others will have to prepare for shifting migration patterns that could leave them virtually deserted.
     
    Today, 1 in 8 people live in urban slums, and by 2025, "it is likely that 1.6 billion will require adequate, affordable housing," according to the UN-Habitat's World Cities Report 2016.

    Behrokh Khoshnevis, professor of engineering at the University of Southern California and director of its Center for Rapid Automated Fabrication Technologies (CRAFT), is one of many innovators looking to ease the housing crises of the future. "Somehow, we have to address the issue of shelter, which is at the base of the pyramid of needs," Khoshnevis said.
     
    He hopes his 3-D printing construction method, which he calls Contour Crafting, will create a way to build homes for a fraction of the current cost. While he can't do anything about the price of land, Khoshnevis said his technology would build a house in a day and cut down on the construction cost by 30 percent.

    A year ago, when Amazon let a homeless shelter for families move into a former motel it owned, it was viewed as a nice but fleeting gesture.
     
    The motel was on a chunk of downtown property where Amazon planned to eventually erect yet another set of sparkling buildings to meet its insatiable need for office space in this city. The hotel would be torn down and the shelter kicked out when that time came.

That’s a hard thing for a lot of people to do."
    In an interview at the current Mary’s Place site owned by Amazon, which was bustling with families returning to the shelter for the evening, John Schoettler, Amazon’s vice president for global real estate and facilities, said the company would spend “tens of millions of dollars” on the design and construction of the shelter’s portion of the building. Amazon will pay the utilities for Mary’s Place, which will occupy the space rent free, although the organization will continue to pay its own staff.

But Nan Roman, president of the National Alliance to End Homelessness, a nonprofit advocacy group in Washington, D.C., said she was unaware of any other private corporation integrating a homeless shelter into its building.

    If part of a hospital stay is to recover from a procedure or illness, why is it so hard to get any rest?

    There is more noise and light than is conducive for sleep. And nurses and others visit frequently to give medications, take vitals, draw blood or perform tests and checkups — in many cases waking patients to do so.

    Some monitoring is necessary, of course. Medication must be given; some vital signs do need to be checked. And frequent monitoring is warranted for some patients — such as those in intensive care units. But others are best left mostly alone. Yet many hospitals don’t distinguish between the two, disrupting everyone on a predefined schedule.

    Peter Ubel understands the problem as both a physician and patient. When he spent a night in the hospital recovering from surgery in 2013, he was interrupted multiple times by blood draws, vital sign checks, other lab tests, as well as by the beeping of machines. “Not an hour went by without some kind of disruption,” said Ubel, a physician with Duke University. “It’s a terrible way to start recovery.”

    It’s more than annoying — such disruptions can harm patients. Short sleep durations are associated with reduced immune function, delirium, hypertension and mood disorders. Hospital conditions, including sleep disruptions, may contribute to “posthospital syndrome” — the period of vulnerability to a host of health problems after hospitalization that are not related to the reason for that hospitalization.

    “In addressing a patient’s acute illness, we may inadvertently be causing harm by ignoring the important restorative powers of a healing environment,” said Harlan Krumholz, a Yale University physician who has been calling attention to posthospital syndrome for several years. “The key to a successful recovery after illness may be a less stressful, more supportive, more humane experience during the hospitalization.”

    It’s an environment that, all too often, seems set up for everyone else’s convenience but the patient’s. To help patients deal with the stresses of hospitalization, sedatives are often prescribed. These medications, including opioids, carry their own risks, such as addiction.

    “Instead, we could make the environment more conducive to rest and reduce the use of sedatives,” Ubel said.

    Solutions aren’t hard to fathom. Ubel listed some in 2013. Hospital workers could coordinate so that one disruption serves multiple needs: a blood draw and a vitals check at the same time instead of two hours apart.
