  • TMAO and Nature vs Nurture - FORD BREWER MD MPH
This is an editorial of the Tang NEJM article, a landmark article re: TMAO and cv inflammation and risk. The first sentence is hard to understand for most. But it contains the most important point. It states that the "Galtonian distinction between" (NATURE VS NURTURE) is "an overly simplistic dichotomy." His point is that genes and environment interact; it's not just a simple nature vs nurture competition. The second point, maybe more important, is "which nature". Our biome carries over 100 times the genetics that we carry. And the gut is the major bionic contributor.

  • Sleepy? It's a risk for Alzheimer's? Sept 2018 - FORD BREWER MD MPH
Adam Spira and others from Hopkins studied 128 patients prospectively for sleepiness and presence of amyloid-beta 16years later. Those with Excessive Daytime Sleepiness and Napping were 3 times more likely to have amyloid deposits.
Beta-Amyloid deposits are linked to Alzheimer's Dementia. These were detected using Pittsburgh Compound B Positron Emission Tomography (PiB PET) scans.
This supports the link of chronic sleepiness as a risk factor for cognitive decline.

  • Muscle Loss (Sarcopenia): Dangerous & Common - FORD BREWER MD MPH
This is far more common than we think! And it's VERY LETHAL!.
    https://www.cambridge.org/core/journals/proceedings-of-the-nutrition-society/article/ageing-well-a-review-of-sarcopenia-and-frailty/A34C02F64079C6EA10D10E1EF9D930AE
This is a video on the above article. It talks about the increasing prevalence and importance of frailty and muscle loss. Frailty is more functional and conceptual, while muscle loss is objective and visual. "Rise & walk" and walking speed are both objectives, easy and popular ways to measure these. Besides falls and cardiovascular risk, survival of surgery and other stressors is greatly dependent on muscle mass and frailty.

  • Keto Mojo Unboxing (& Why Ketones Matter) - FORD BREWER MD MPH
I got my Keto Mojo device today. I unboxed and used it. Then I did it again and recorded it. I got 3.5 and 4.0 mmol/l for my values. That's usually seen on days 3-5 of water fasting. Why did I see it so early? I'm only 1 day into my fast. I've been doing 60 -72 water faster once each week for a month. And I stay low carb. I've lost about 10 pounds and feel great. So my body is already into a major fat-burning episode.
The Keto Mojo gets good reviews from John and others. And ketosis is important: it shows you're burning fat.

  • Is Mom's Doc Missing Her Malnutrition Risk? - FORD BREWER MD MPH
This is the 3rd in a series on looking at easily missed, but important disease processes. This one is on malnutrition. I've done a couple of related videos - "google Ford Brewer youtube sarcopenia". Sarcopenia is muscle wasting, by the way.
Even though we think of this as simply, "a thin, little old man - or lady". This is actually very dangerous. Medicare rates it as one of the biggest predictors of hospitalization, disability & death.
Things to think about including more than just a low BMI (less than 19). They also include loss of weight(without intentional dieting) 5% within a month, or 3% in a week. The doc should check pre-albumin, not albumin.

  • Stem Cell for the KNEE:What is it? Does it Work? - FORD BREWER MD MPH
This is a Nov 2017 Literature review re: stem cell and PRP treatment of the knee. Knee injury is one of the most common orthopedic causes of disability. And it's growing with the aging and obesity epidemics. There are 2 major competing techniques: PRP and BMAC.
PRP is Platelet Rich Plasma is common with new standards of over 5 mil platelets/ml of plasma. The attraction of platelets is actually the growth factor contained by the platelets.
BMAC is Bone Marrow Aspirate Concentrate. This is usually obtained from the backside of the pelvic bone in a sterile environment. It is minimally processed, simply spun down.
They repeat the general theme. There is a lot of promise, but research seems to be going too slow. Here are some reasons: 1. outcomes are subjective (pain ); 2. their processes are changing (taking out WBC and adding scaffolding); and 3. failure to standardize and document procedures.

  • New England Journal re: Smoking Cessation, Weight Gain & Health - FORD BREWER MD MPH
This is a New England Journal Article re: smoking cessation, weight gain, and health.
    https://www.nejm.org/doi/10.1056/NEJMoa1803626
Yes. Stopping smoking tends to make you gain weight. And yes, the weight gain even increases the risk for diabetes. But the smoking cessation has twice the impact on your health as weight gain and diabetes. There's no question - STOP SMOKING! if you want to be healthy.
The "ill quitter" effect is the known research confusion caused by people that stop smoking due to cancer or lung disease. It was studied and shown to have some impact. But attempts to remove this impact were challenging.
