Do you know what happens in your body when you eat a meal? Or did you, like most of us, make it to adulthood without anyone explaining the basics?

If that question made you pause, you're not alone. Most "healthy eating" advice arrives in fragments — a rule from one diet trend, a warning from another — without ever adding up to a full picture of how food actually works in the body.

Protein, carbs, fat, vitamins, minerals, phytonutrients: these aren't competing categories to master one at a time. They're a system. Once you understand how the pieces fit together, meal planning stops feeling like guesswork and starts feeling like common sense.

This episode covers exactly that — what each macronutrient is doing for you, why micronutrients matter even in small amounts, and how to build a plate that supports your body without obsessing over any single food or number. No fad diets, no miracle foods. Just the fundamentals, explained clearly.

    Topics discussed:

    • Macronutrients: The Building Blocks of Your Diet
    • Protein: Why Your Body Needs It
    • Carbohydrates: Choosing Better Sources of Energy
    • Fat: Why It's Essential for Good Health
    • Caloric Density: Why Some Foods Contain More Calories Than Others
    • Micronutrients: The Vitamins and Minerals Your Body Needs
    • Whole Foods: Why Variety Matters More Than Individual Nutrients
    • Building a Balanced Plate for Better Nutrition
    • Reducing Added Sugar Without Feeling Restricted
    • Eating More Protein to Stay Fuller Longer
    • Focus on Adding Healthy Foods Instead of Restricting Them
    • Creating Healthy Eating Habits That Last

Your plate is the one thing you control every single day — three times a day, for the rest of your life. Every meal is a chance to give your body what it actually needs instead of guessing, restricting, or chasing whatever the latest trend says to avoid. This episode hands you the framework to make that choice easier, meal after meal. Press play and find out how to make it work for you.

Have You Ever Wondered If Your Telehealth Weight Loss Clinic Is Actually Giving You Real Medical Care, or Just a Prescription?

This episode dives into a JAMA study that's been generating serious buzz throughout the obesity medicine community, and once you hear what it uncovered, you'll understand exactly why. Researchers conducted a "secret shopper" investigation, sending someone posing as a patient to 49 different telehealth clinics offering obesity management medications, simply to see what kind of care they'd actually receive. The findings shed light on where the gaps really are in this rapidly expanding area of healthcare, and they back up what a lot of obesity medicine providers have suspected for quite some time.

We're unpacking what comprehensive obesity care is truly supposed to look like, and how you can figure out whether the care you're getting, or the care you're offering, actually holds up. And there are two calls to action woven throughout: if you're someone taking these medications or considering starting them, we'll walk through how to find a provider who's doing this the right way. And if you're a clinician providing this kind of care already, consider this your reminder to put yourself out there more, because there are patients who genuinely need access to someone doing it well.

    Topics Discussed:

    • The JAMA Study Raising Questions About Obesity Telehealth
    • The Missing Piece in Many Telehealth Weight Loss Programs
    • Convenience Shouldn't Replace Comprehensive Medical Care
    • The Importance of Clinical Oversight in Telehealth Prescribing
    • What Comprehensive Obesity Care Should Look Like
    • Where to Start If You're Considering Weight-Loss Medication
    • How to Find High-Quality Obesity Care

This episode covers a lot of ground, from the specific numbers behind the study to what a truly thorough evaluation should include, and what red flags to watch for if you're navigating this kind of care yourself. Whether you're a patient trying to figure out if your provider is doing right by you, or a clinician who wants to understand where the industry is falling short, this conversation breaks it all down in a way that's genuinely worth your time. Press play to catch the full episode and get every detail we covered.

  • Cut Back Screen Time to Improve your Health and Weight

What if the biggest thing standing between you and your health goals wasn't food, or exercise, but the screen you're holding right now?

Today we're talking about screen time — how too much of it affects your health, your weight, and what you can actually do about it. I've got five problems screens create, and five solutions to go with them, so stick with me.

Screen time isn't something I bring up with patients as often as I probably should, but it shows up in two specific places in my practice. The first is during a comprehensive annual wellness visit, where I think about screens the same way I think about substances like alcohol, tobacco, or marijuana — something we tend to overuse, often to numb out or fill a void, and something we can usually cut back on to genuinely improve our health. The second is in weight maintenance. Once a patient reaches their goal weight and we shift into maintenance mode, I walk through a handout built around the National Weight Control Registry — a long-running study tracking people who lost 30 or more pounds and kept it off for at least a year. One of the most telling data points from that registry: 62% of people who successfully maintained their weight loss were watching less than 10 hours of TV a week. That's still over an hour a day, so there's plenty of room for your shows — but it tells us something real about the role screens play in long-term success. Today we're getting into exactly why.

    Topics discussed:

    • The Sneaky Way Screen Time Is Draining Your Daily Movement
    • Why Screen Time and Snacking Go Together More Than You Realize
    • The Hidden Link Between Blue Light, Sleep, and Weight
    • The Stress Connection Nobody Talks About With Screen Time
    • The Real Cost of Letting Screens Eat Your Free Time
    • One Easy Shift That Actually Cuts Down Screen Time
    • Try the 30/30 Rule to Break Up Long Periods of Sitting
    • Keep Screens Out of Mealtimes
    • Set a Screen Curfew Before Bed
    • Pair Screen Time With Movement

There's a lot packed into this one, and every piece builds on the last. We'll get into why your favorite show might be quietly killing your daily movement, how it sneaks into your snacking and sleep without you noticing, and how something as small as the 30/30 rule or a bedtime curfew can start turning that around. Press play and find out where your screen time might be costing you more than you think.

Ever wonder why losing weight can suddenly feel impossible in your 40s or 50s, even when nothing about your diet or workout routine has changed?

That's exactly the question this week's episode sets out to answer, with guest Dr. Angela Glasnapp joining the conversation. Dr. Glasnapp is a board-certified bariatric surgeon, obesity medicine specialist, and menopause practitioner with New York Bariatric Group, and for more than 16 years, she's helped patients improve their health through evidence-based treatment for obesity and metabolic disease, blending surgical, medical, and lifestyle approaches into one comprehensive strategy. As her practice evolved, she noticed a significant gap in care for women navigating perimenopause and menopause, which led her to bring that expertise into her practice too.

    In this episode, we cover:

    • The First Step to the Right Treatment Is Just Getting the Full Picture
    • Bariatric Surgery Eligibility, Explained
    • Can Bariatric Surgery and Weight Loss Medications Be Used Together?
    • Understanding the Weight Set Point Theory
    • Why Dr. Glasnapp Expanded Her Practice to Menopause Care
    • How Hormone Therapy and GLP-1 Medications May Work Together
    • A Comprehensive Approach to Women's Health

Dr. Glasnapp breaks down everything from choosing the right treatment path to the science behind weight regulation, and why menopause deserves a much bigger seat at the table in obesity medicine.

This episode is packed with the kind of info that could totally change how you think about weight, hormones, and what "stuck" really means. Tap play and let it all sink in, one lightbulb moment at a time.

Ever feel like exercise gets treated as this magic weight-loss tool, when really that's not even its main superpower?

Here's the truth: most people don't lose significant weight from exercise alone. Your body is smart (annoyingly so) — it adapts, burns fewer calories than you'd expect, and can even nudge you to move less throughout the day or eat a little more to compensate. So if you've ever felt discouraged because the scale didn't move despite all your effort at the gym, you're not doing anything wrong. That's just physiology doing its thing.

But here's the part that matters even more: exercise isn't about the scale, and it never should be. It's genuinely one of the most powerful things you can do for your overall health, no matter your weight. It also plays a major role in maintaining weight loss once you've achieved it, arguably a bigger win than losing the weight itself. Exercise was never meant to be a punishment for how you look, it's a tool for feeling stronger, sleeping better, and building a body that can support you for the long run.

This week's episode dives into five new conversations we should all be having about exercise, from the real sweet spot for strength training to the surprising ways your muscles work behind the scenes:

    Topics discussed:

    • The Sweet Spot for Strength Training, According to Research
    • The Case for Mixing Up Your Workouts
    • Muscle Mass vs. Muscle Function: What Actually Matters on GLP-1s
    • The Minimum Effective Dose: How Little Exercise Actually Counts?

What If Your Weight Struggle Was Never About Willpower?

What if the reason some people struggle with their weight their entire lives has nothing to do with what they're eating or how hard they're trying — and everything to do with their DNA? It's a question that doesn't get asked nearly enough, and today, we're finally diving into it.

For so many people living with severe obesity, the missing piece of the puzzle isn't a better diet plan or more motivation. It might be written right into their genetic code — and that changes everything about how it should be understood and treated.

Today's guest is Dr. Jesse Richards — board-certified in internal medicine and obesity medicine, Director of Obesity Medicine at the University of Oklahoma School of Community Medicine, and the person his colleagues call when they have questions about the genetics of obesity. His research covers genetic obesity, GLP-1s, and novel treatments for obesity and substance use disorders. Safe to say, he knows this space better than almost anyone.

    In This Episode, We Cover:

    • What Sparked Dr. Jesse's Interest in Genetic Obesity
    • Why Some People Are Born With a Stronger Genetic Drive Toward Obesity
    • The Genetic Syndromes Behind Severe Obesity That Most People Have Never Heard Of
    • It's Not in Your Head — But It Is in Your Biology
    • Why Genetic Test Results Are Only Part of the Story

This is just the surface — give the full episode a listen to get into everything we covered and more.

What if Medicare finally made GLP-1 medications for weight management more affordable — could this be the moment that changes access for millions of people?

Today, we're talking about something that's been years in the making in the world of obesity care: the expanded coverage of anti-obesity medications for patients on Medicare. Starting July 1st, eligible Medicare beneficiaries have a real opportunity to access GLP-1 receptor agonist medications at a much more affordable monthly cost through a new federal program — and for many patients, this could mark a major shift in access to treatment that's historically been limited or completely out of reach under Medicare's coverage rules.

This change didn't happen overnight. It comes after years of advocacy, policy discussions, and ongoing efforts to get obesity recognized as a chronic medical condition that deserves treatment just like any other disease. In this episode, we break down where this program came from, how it connects to the bigger picture of obesity care, who it applies to, and everything you need to know about eligibility, covered medications, and how the process actually plays out in real life.

    Topics discussed:

    • How the Bridge Program Could Expand Access to Obesity Medications
    • For the First Time, Medicare Is Opening the Door to Obesity Medications
    • The BMI and Health Condition Criteria
    • Why the Reason for Your Prescription Matters Just as Much
    • The Details That Make a Real Difference — Prior BMI and Lifestyle Support
    • The List of Weight Loss Medications Covered by the Bridge Program

Want the full breakdown? This episode covers all the details on eligibility, covered medications, and what this program could mean for you or someone you love navigating Medicare and obesity treatment. Find it wherever you get your podcasts — and if it helps, share it with someone who might need to hear it too.

What if the reason so many people struggle on GLP-1 medications isn’t the medication itself — but the way it’s being dosed?

Today we’re diving into something that could genuinely change the way you think about GLP-1 therapy: custom dosing. And there is honestly no one better to talk about this with than Dr. Ian Justl Ellis.

Dr. Ian is a board-certified physician, certified personal fitness instructor, and the creator of the MyLevel GLP-1 Precision Dosing Engine. He’s also the founder and CEO of VOA Fit, where he provides highly personalized concierge care through advanced GLP-1 therapy, hormone optimization, and peptide protocols — all paired with real-world fitness and nutrition strategies. Before transitioning into obesity medicine, he spent a decade in emergency medicine.

What makes his perspective especially valuable is how he bridges clinical expertise with lived experience in weight management. His work doesn’t just focus on prescribing GLP-1s, but on optimizing how they’re actually used in real-world patients. And that’s exactly what this conversation is about — how dosing strategy can completely change outcomes.

    Topics discussed:

    • Why GLP-1 results may depend more on dosing than the medication itself
    • Dr. Ian’s early belief in discipline — and why it eventually broke down
    • When personal struggle turned into a medical reframe of obesity
    • The first experience with GLP-1s and the reality behind the results
    • Why “dose” isn’t the full story — understanding GLP-1 drug levels
    • Moving from standard protocols to individualized GLP-1 leveling
    • How personalized GLP-1 dosing works in real patients
    • The future of GLP-1 therapy: a more responsive, guided model of care
    • Rethinking weight loss: from fixed doses to adaptive biology

There’s a lot more nuance to GLP-1 therapy than standard protocols tend to show, and this episode really breaks down what happens when dosing becomes personalized rather than fixed.

So if you want to understand how this approach actually works in practice — and why it might completely shift how we think about weight management — tune in to the full episode.

What If the Lowest Dose Isn't Actually the Safest Dose?

If you've spent any time in weight loss or wellness spaces online, you've probably come across the term microdosing — and chances are, it was framed as the smarter, gentler, more mindful way to use GLP-1 medications. But is that actually true? That's exactly what we're digging into today.

    In this episode, we cover:

    • Is "Microdosing" GLP-1s Actually a Thing? — Where the term came from, why it took off, and whether it holds up clinically.
    • The Right Dose Is the One That's Actually Working for You — What therapeutic dosing actually means and how to know if yours is doing its job.
    • Weight Loss Is a Side Effect, Not Always the Goal — Why the scale isn't the only thing worth tracking — and what to measure instead.
    • There Is No Prize for the Lowest Dose — The real cost of staying subtherapeutic and why "less is more" doesn't always apply here.

There is no prize for taking the lowest dose. The goal isn't to stay as low as possible, and it isn't to push as high as possible either. The goal is to find the dose that actually works for you — for your body, your health conditions, and your specific outcomes. Check out the full episode and give it a listen — it's worth your time.

Link: Weight Care Guide — Dr. Francavilla Show (thedrfrancavillashow.com)

Have you ever wondered if counting calories is actually worth your time — or if it's just another diet trap dressed up in a tracking app? In this week’s episode, we talk about just that, and the answer is more nuanced than a simple yes or no. Calories are real, they matter, and paying attention to them can be genuinely useful — but the way many people approach calorie counting is often where things go sideways.

Here's the thing though — calories are calculated using a  lab device. Which means it doesn’t take into account how the body actually processes those calories. You digest food, respond to it hormonally, and process it completely differently depending on what you're actually eating, not just how much. Protein burns some of its own calories just through digestion. Fiber doesn't get absorbed at all. And the same 100 calories from apple juice versus a whole apple with peanut butter will hit your body in completely different ways — one spikes your blood sugar and leaves you hungry again fast, the other keeps you full and satisfied for hours. Same number on paper, totally different story inside your body.

That's why the old "calories in, calories out" model, while not entirely wrong, is a pretty significant oversimplification. Your metabolism adapts, your hunger hormones shift, and the quality of what you eat plays just as important a role as the quantity. Calories are one important piece of the puzzle — but they're far from the whole picture.

    In this episode, we cover:

    • Why Not All Calories Are Created Equal
    • Macros, Metabolism, and Why Your Body Isn't a Machine
    • Log Like a Scientist, Not a Judge
    • When You've Already Done the Work
    • Ditch the "I Blew It" Mentality

There's a lot more where this came from — tune into the full episode and you might just walk away thinking about food completely differently.
