  • Understanding CV Risk: Low Carb: Remaining Questions: Lancet Article - FORD BREWER MD MPH
There are several remaining questions to the carb diet issue:
    1. is the U-curve real? There is a U-curve that usually inflects at 51%. Most people surveyed get 51% of their calories from carbs.
Other dietary recall studies indicate the same - 51%, even the PURE study.
According to the ARIC follow-up published in Lancet Aug 2018, the lifespan decreases as the % of carbs drops. According to the PURE study, higher levels of carbs showed decreasing lifespan. Why?
Is Glycemic Index, or Glycemic Load involved in the explanation? Is it the quality of the carbs?
    2. Also, what happened in the ARIC follow-up to the group that "substituted lower carbs with plant protein and fat"? This subgroup, according to the study also lived longer. It was described in the text and there was a separate table. But it didn't show up in the U curve. Why?

  • Omega-3 Index for Measuring EPA and DHA - FORD BREWER MD MPH
This video discusses the Omega-3 Index and how to evaluate our EPA, DHA, and other fatty acid levels in our blood.
About John Lorscheider - John is a frequent contributor and has developed and manages our Forum. He’s also a PrevMed patient. Like most of us, John’s family and personal history include significant cardiovascular risk.
About John Lorscheider - John is a frequent contributor and moderator of our YouTube channel. He’s also a PrevMed patient. Like most of us, John’s family and personal history include significant cardiovascular risk. He also developed the PrevMed Community Forum at https://759241a66b89-003789.vbulletin.net/
John came from a technical background and spent his entire career in providing engineered fluid process systems and equipment to industry, as well as performing root cause failure analyses for his customers.
His own study and research made it clear he wasn’t getting the whole story about prevention from his docs. They just didn’t know. So he’s been in several roles of running prevention medical groups to help further the science.

  • Low Carb - GLYCEMIC INDEX from Dietary Recall - FORD BREWER MD MPH
This is the second in a series on Low Carb Dietary Recall Research. Glycemic Index is the impact on blood sugar by foods. The Glycemic Load measures the impact of a meal. Daily Glycemic load would cover and total all food during the day.
Standardized software ( The NDSR - Nutrition Data System for Research) is used by nutritionist interviewers to find matches to the foods. After the food matches are made, the glycemic index must be calculated. That's done using other standard databases (the International Table of Glycemic Index and Glycemic Load Values and the University of Sydney online databases).
After these data are found, calculations have to be made. The frequency and amount of foods ingested must then be calculated. Findings were blinded and replicated. Repeat calculations were found to have a high correlation.

  • Kidney Disease & AGE(Advanced Glycation Endproducts)-FORD BREWER MD MPH
Lee Hull is a kidney patient. He's writing a book called STOPPING KIDNEY DISEASE. The book is pretty good. It has a great focus on prevention.
He's got a great list of dietary AGE. AGE are proteins (& fats) that are bound covalently (strongly bonded) to glucose or other sugars. Dietary AGE comes mostly from seared meats. Plant sources and uncooked foods have few AGEs.
Unfortunately, he doesn't mention the major source of AGE - hyperglycemia. HgA1c, the well-known marker of blood glucose levels over monthly periods - is actually an AGE. It's the AGE of the hemoglobin molecule. It gets bonded to Glucose in the blood when it is high.

  • Diabetic Glucose Spikes in "Healthy" Adults pt 2- FORD BREWER MD MPH
This is part 2 of a series on Diabetic Glucose Spikes Seen in "Healthy Adults". This video reviews PLOS Biology. Micheal Snyder, head of the genetics department at Stanford, and Heather Hall, a graduate student. They used Continuous Glucose Monitor (CGM). 15% of otherwise "healthy" adults had prediabetes. 2% met the criteria for full-blown diabetes. Age and BMI were major drivers of high variability patterns.

  • Carbohydrates & Fatty Liver - Fact or Fiction - FORD BREWER MD MPH
This video covers the Sevastianova study from the Minerva Foundation in Helsinki. It was published in the American Journal of Clinical Nutrition. It's title Effect of short-term carbohydrate overfeeding and long-term weight loss on liver fat in overweight humans.

Basically, they added 1000 kcal of simple carbs to the diet of 16 obese individuals. It measured serum indicators of de novo lipogenesis, MRI for liver fat content, and body weight.
They found a 27% increase in liver fat after 3 weeks of adding 1000 kcal of simple carbs. After 6 months of decreased calories and carbs, the liver fat decreased again.

  • Aged Garlic Extract - Reversing CV Inflammation? (Budoff Study) - FORD BREWER MD MPH
John Lorscheider took Aged Garlic Extract (AGE) based on research he'd seen by Matthew Budoff. John said he got no significant impact on LP-PLA2 until he lost 20 pounds, cut his carbs, and started low-dose Rosuvastatin.
This video reviews the article. It was small (65 total participants) and on intermediate-risk participants (male firefighters). He saw a decrease in hsCRP and a slowing of Coronary Artery Calcification (CAC). This combination in a randomized clinical trial could be a strong signal. But he also saw a drop in the BMI of the study group. I looked again at the study to see if there were randomization irregularities or a discussion of this potential confounder. I could not find anything.
